The Role
As a Care Coordinator, you will be responsible for coordinating care services, ensuring clients receive safe, effective, person-centred care while supporting our team of care professionals. You will work closely with the Registered Manager, Deputy Manager, Team Leaders, Care Staff, Clients, Families, Local Authorities, NHS professionals and other healthcare partners.
The successful candidate will ensure continuity of care, maintain compliance with CQC regulations, manage staff rotas, respond to emergencies and provide excellent customer service to clients and stakeholders.
Key ResponsibilitiesCare Coordination
- Coordinate and schedule domiciliary care visits efficiently.
- Produce and maintain staff rotas to ensure all care calls are covered.
- Match carers to clients based on skills, experience, personality and client preferences.
- Arrange emergency cover for sickness, annual leave and unforeseen absences.
- Monitor missed, late or incomplete care calls and take immediate action.
- Ensure continuity of care by allocating regular carers wherever possible.
- Liaise with care staff regarding daily schedules and changes.
- Monitor capacity and identify recruitment needs.
Client Management
- Act as a first point of contact for clients and their families.
- Build positive professional relationships with service users.
- Respond promptly to concerns, complaints and compliments.
- Support the onboarding of new care packages.
- Assist with client assessments, reviews and care planning.
- Ensure care packages remain suitable and meet changing needs.
- Liaise with local authorities, Integrated Care Boards (ICBs), social workers, district nurses, GPs, occupational therapists and other healthcare professionals.
- Support hospital discharges and urgent care package requests.
Staff Support
- Provide daily support to care workers.
- Communicate rota updates effectively.
- Record and report staff availability.
- Support induction of new care staff.
- Assist with supervision scheduling.
- Monitor staff training compliance.
- Monitor probation reviews.
- Ensure staff hold valid DBS, Right to Work and mandatory training.
- Promote staff wellbeing and engagement.
- Support investigations, disciplinary meetings and return-to-work processes when required.
- Support with staff Spot checks
Compliance & Quality Assurance
- Ensure services comply with CQC Fundamental Standards.
- Maintain accurate electronic records.
- Monitor electronic call monitoring systems.
- Review care documentation for quality and completeness.
- Ensure medication documentation is accurate.
- Escalate safeguarding concerns immediately.
- Report incidents and accidents appropriately.
- Ensure confidentiality is maintained at all times.
- Assist with audits including:
- Medication audits
- Care plan audits
- MAR chart audits
- Spot checks
- Staff file audits
- Client documentation audits
Administration
- Maintain accurate client and staff records.
- Update electronic care management systems.
- Process referrals.
- Prepare documentation for new care packages.
- Maintain confidentiality in accordance with GDPR.
- Answer telephone calls and emails professionally.
- Produce reports when required.
- Assist with invoicing information where necessary.
- Order office supplies.
- File documentation accurately.
Communication

Excellent communication skills are essential, with the ability to remain calm under pressure while managing competing priorities.
Essential Requirements
- Previous experience as a Care Coordinator, Senior Carer or similar role within domiciliary care.
- Good knowledge of CQC regulations and domiciliary care standards.
- Experience creating and managing staff rotas.
- Excellent organisational skills.
- Strong administrative skills.
- Excellent telephone manner.
- Confident using Microsoft Office.
- Experience using electronic care management systems.
- Ability to work under pressure.
- Excellent problem-solving skills.
- Ability to prioritise workload.
- Professional and compassionate approach.
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
- Full UK driving licence .
Desirable Experience
- NVQ Level 3 in Health & Social Care (minimum).
- NVQ Level 5 (desirable).
- Experience within a CQC-regulated domiciliary care service.
- Knowledge of PASS, or similar care management software.
- Experience managing complex care packages.
- Experience working with NHS professionals and commissioners.
- Understanding of safeguarding legislation.
- Knowledge of GDPR and confidentiality requirements.
- Recruitment and onboarding experience.
- Experience supporting CQC inspections.
