Ark Royal Ward based at Gosport War Memorial Hospital has exciting positions available for Registered Nurses with an interest and passion for caring for the over sixty fives. We are looking for an enthusiastic, motivated and well organised staff.

Main duties, tasks & skills required
The post holder will be required to:

- Person-Centred Care
- Participate in the holistic assessment, implementation and evaluation of programmes of care and therapy for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs. Other settings include, residential homes, nursing homes, clinic's community centres and community hospitals.
- Recognise and respond to the need for appropriate referral to other members of the Community Care Team or external organisations including appropriate escalation of potential Safeguarding risks.
- Provide safe medicines management in your area, according to professional and organisational guidelines to ensure safety and efficacy.
- Inform the Senior Community Nurse / Community Matron of any concerns with patients, relatives, visitors, or staff that may compromise patient care
- Develop and maintain constructive working relationships and liaise effectively with all members of the multidisciplinary team so that patients' needs are met
- Communicate effectively with colleagues, patients and carers so that information is shared in order to meet patients' needs.
