Twenty Fifth Avenue Housing is seeking an experienced and organised Referral Officer to join our Birmingham operations.
You will play a key role in receiving, assessing and onboarding referrals into our housing services, working with partner organisations including local authorities, probation services, the NHS and charity partners.
This is an excellent opportunity for someone with experience in supported housing, social care or homelessness services who is confident carrying out assessments, managing sensitive information and building effective relationships with referral partners.
About the Role
As Referral Officer, you will manage referrals from initial receipt through assessment and onboarding, helping to ensure prospective tenants are appropriately assessed for our available provision.
You will undertake risk screening, maintain accurate referral and case records and coordinate successful applicants' handover to our housing and support teams.
The role also involves developing positive working relationships with existing and prospective referral partners across Birmingham.
Key Responsibilities
- Receive and process referrals from local authorities, probation services, NHS services, charities and other partner organisations.
- Assess referrals and undertake appropriate risk screening to determine suitability for available accommodation.
- Coordinate the onboarding of new tenants and ensure an effective handover to housing and support colleagues.
- Maintain accurate, confidential and up-to-date case notes and referral records.
- Build and maintain effective relationships with referring organisations and external partners.
- Ensure safeguarding and confidentiality requirements are followed throughout the referral and onboarding process.
About You
We are looking for someone who can combine strong assessment skills with excellent communication, sound judgement and a person-centred approach.
Essential
- Experience working within supported housing, social care or homelessness services.
- Strong assessment and risk-screening skills.
- Good understanding of safeguarding and confidentiality requirements.
- Excellent written communication skills, particularly for case notes and professional correspondence.
- Ability to communicate effectively with applicants, colleagues and external partners.
Desirable
- Established relationships with Birmingham referral partners.
- Experience using referral or case-management systems.
- Full UK driving licence.
