Vision Care is looking for an articulate, enthusiastic and out-going Care Coordinator to join our office team
You will be a well presented individual with a minimum of 12 months experience in care
Have an excellent telephone manner
We are seeking an energetic care co-ordinator to join our very busy office team
You will be dedicated to efficient and effective collaboration between care teams, service users and their families.
Must be outgoing, professional and have a flexible approach to reaching mutual beneficial resolutions
You will be working as part of an already established care- coordination team daily tasks will include
-Growth of business by networking and acquiring new avenues of work
-addressing service user queries both on the phone and face to face
- on -call support (one week every month roughly)
-establish and develop relationships with other health care professionals
-assess individuals needs in the community and complete a full assessment in person
Duties will include:
· Making person centered care plans for our service users
· Ensuring quality of care plans is up to QA standards set by Vision Care
· Plan the deployment of resources to ensure the appropriate amount of support is provided to meet the needs of service users.
· Allocate resources to meet service user’s needs.
· Provide managers, carers and service users with information and guidance to ensure effective service delivery.
· Record and document the allocation of resources to meet service user’s needs.
· Provide administrative support to managers in relation to service delivery, human resource management, health and safety and risk assessments.
· Assist managers with the provision of management information.
· Ensure records (both manual and computerised) are maintained in accordance with administrative procedures.
· Support with reviewing and auditing all service user files
· Participate in the implementation and subsequent maintenance of computerised administrative systems.
· Attend meetings as requested by the appropriate manager.
· To undertake training as necessary in relation to the role.
· Commitment to work a cover shift pattern including one weekend a month and on-call overnight support as rostered

candidate must be computer literate and experience of using Care Planner would be beneficial
