Responsibilities
- Develop, implement, and regularly update comprehensive care plans tailored to each resident’s unique needs, including medical, emotional, and social aspects
- Coordinate with healthcare professionals to ensure proper medication administration and adherence to treatment protocols
- Monitor residents’ behavior and condition, employing effective behavior management techniques to promote well-being
- Provide direct patient care such as assistance with activities of daily living, mobility support, meal preparation, and light housekeeping tasks
- Conduct regular assessments to evaluate progress and adjust care strategies accordingly
- Maintain accurate documentation of care activities, medication records, and incident reports in compliance with regulatory standards
- Support the management team as part of the on-call rota
Qualifications
- Proven experience in nursing home settings or senior care environments with a strong understanding of dementia and Alzheimer’s care
- Certification or training in first aid is highly desirable
- Knowledge of medical terminology and case management principles to coordinate effective care delivery
- Experience with assisted living facilities or home care services is preferred
- Ability to perform heavy lifting safely when assisting residents with mobility or transfers
- Valid driver’s license with reliable transportation for community visits or appointments
- Excellent communication skills to collaborate effectively with residents, families, and healthcare teams
