We are seeking compassionate and dedicated Community Care Assistants and a Care Coordinator with experience in home care to join our team. Duties
- Deliver personalised care to clients within their homes, assisting with daily activities such as personal hygiene, meal preparation, and medication management
- Develop, implement, and review individualised care plans in collaboration with clients and their families
- Supervise and support other care staff where applicable, ensuring the delivery of consistent and high-quality care
- Maintain accurate documentation of care provided, adhering to organisational policies and procedures
- Coordinate appointments, liaise with healthcare professionals, and ensure smooth communication between all parties involved
- Drive between clients’ homes as required, ensuring timely attendance at scheduled visits
- Monitor client progress and report any concerns or changes in condition promptly to relevant health professionals
- Uphold the dignity, privacy, and independence of each individual while providing compassionate support
Skills
- Valid driving licence with reliable access to a vehicle for client visits
- Experience working within care homes or senior care environments is highly desirable
- Supervising experience or leadership skills that enable effective team coordination and mentorship
- Strong communication skills to interact confidently with clients, families, and healthcare professionals
- Ability to develop and manage care plans tailored to individual needs
- Proficiency in IT systems for documentation and communication purposes
- Compassionate approach with excellent organisational skills and attention to detail
- Ability to work independently while maintaining a team-oriented attitude
