Care Coordinator – Domiciliary Care (CQC Registered)
Gap Kare is looking for an experienced Care Coordinator to join our domiciliary care team.
- Shift pattern: Rota-based, 3 shifts per day covering 6am–10pm (early, mid, and late cover)
- Setting: Domiciliary (home care) – CQC compliant service
- What we need from you:
- Solid experience coordinating care in the domiciliary/home care sector
- Confident managing on-call duties and reporting incidents/concerns in line with CQC requirements
- Strong understanding of care planning, rota management, and safeguarding processes
- Comfortable handling carer queries, family liaison, and last-minute schedule changes
- Good written and verbal communication for accurate handovers and reporting
What the role involves:
- Coordinating carer rotas across a 6am–10pm split-shift pattern, ensuring full coverage each day
- Acting as the on-call point of contact during assigned shifts, escalating and reporting appropriately
- Supporting CQC compliance through accurate record-keeping and timely reporting
- Working closely with the care team to maintain continuity and quality of care for service users
