Care Coordinator – Job Overview
We are a fast-growing domiciliary care company looking for an experienced, organised, caring and reliable Care Coordinator to join our expanding team. Previous experience in domiciliary care and care coordination is essential.
The Role
The main focus of this role will be rota management and coordination of our care team. You will be responsible for creating and managing the daily rota, ensuring all visits are covered, arranging cover for sickness and annual leave, and responding quickly to changes and last-minute issues.
You will communicate regularly with carers, clients and families to ensure visits run smoothly and any issues are dealt with promptly. You will also liaise with brokerage teams, hospital discharge teams, social workers and other professionals to coordinate new care packages and changes to existing care.
You Must Have
- Previous experience in domiciliary care and care coordination – essential.
- Experience managing and coordinating staff rotas.
- A good understanding of the challenges involved in domiciliary care.
- Excellent organisational and time-management skills.
- The ability to remain calm and make decisions under pressure.
- A good grasp of both written and spoken English.
- Good communication and problem-solving skills.
- The ability to work independently and use your own initiative.
- A flexible approach and the ability to respond to last-minute changes.
