- Coordinate and manage client care services
- Schedule and coordinate appointments, referrals, and follow-up visits
- Communicate with healthcare providers, client, and their families to ensure continuity of care
- Assist in the development and implementation of care plans
- Maintain accurate and up-to-date client records
- Provide support and education to patients and their families regarding healthcare services and resources

- Proven experience working in a healthcare setting or similar role in an expanding business.
- Strong organizational and time management skills
- Excellent communication and interpersonal skills
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Ability to multitask and prioritize tasks effectively
- Knowledge of medical terminology
