TDDI REGISTERED NURSE / COMPLEX CARE NURSE
Location: Hybrid / Remote with occasional office and service visits
Hours: Initially 10 hours per week
Rate: £40 per hour (£400 per week based on 10 hours)
Contract: Part-time initially, with potential to progress to a full-time position following CQC registration

About the Role
We are seeking an experienced and motivated NMC Registered Nurse to support a domiciliary care provider that is expanding its services to include Treatment of Disease, Disorder or Injury (TDDI) and complex clinical care within the community.
The organisation is currently preparing for the extension of its CQC registration. During this initial registration and setup period, the successful nurse will work approximately 10 hours per week, primarily supporting the development of the clinical governance, policies, procedures and systems required to safely deliver nurse-led complex care.
The initial work can largely be completed remotely, although occasional attendance at the office or service location may be required.
Once CQC registration for the expanded service has been successfully completed, there will be an opportunity to discuss progression into a full-time clinical role, supporting and overseeing complex care packages within the community.
Initial Responsibilities – CQC Registration & Service Setup
During the initial 10-hour-per-week setup phase, responsibilities will include:
- Supporting the organisation with preparation for its CQC registration/variation to deliver TDDI-regulated activities.
- Developing and reviewing clinical policies, procedures and protocols for complex care delivery.
- Supporting the development of robust clinical governance systems.
- Developing clinical risk assessment and risk management processes.
- Creating and reviewing care planning and clinical assessment documentation.
- Developing procedures for medication management and administration.
- Supporting the development of infection prevention and control procedures.
- Establishing systems for clinical incident reporting, escalation and learning.
- Developing competency frameworks for care staff undertaking delegated healthcare tasks.
- Supporting the development of training and competency assessment processes.
- Ensuring clinical documentation reflects current professional standards, NMC requirements and CQC expectations.
- Working closely with the Registered Manager and senior leadership team during the registration and service-development process.
Responsibilities Following CQC Registration
Following successful registration and the commencement of complex care packages, the role is expected to expand to include:
- Completing comprehensive nursing assessments for individuals receiving complex care in their own homes.
- Developing and reviewing individualised clinical care plans.
- Completing clinical and environmental risk assessments.
- Providing clinical oversight of complex care packages.
- Supporting individuals with a range of complex healthcare needs.
- Liaising with GPs, community nursing teams, hospitals, specialist clinicians and other multidisciplinary professionals.
- Providing clinical advice and guidance to care and support staff.
- Training, assessing and signing off staff competencies for delegated clinical tasks.
- Monitoring the quality and safety of clinical care delivered within the community.
- Reviewing incidents, medication errors, safeguarding concerns and clinical risks.
- Supporting audits, quality assurance and continuous service improvement.
- Ensuring care remains person-centred, evidence-based and compliant with CQC and NMC standards.
Depending on the needs of individual service users, clinical oversight may include areas such as tracheostomy care, PEG/enteral feeding, ventilation support, catheter and stoma care, epilepsy management, diabetes management, medication administration, wound care, palliative care and other complex clinical interventions.
Essential Requirements
The successful candidate must:
- Be a Registered Nurse with current NMC registration.
- Have experience working with individuals with complex healthcare needs.
- Have experience providing or overseeing clinical care within domiciliary, community or home-based settings.
- Have a strong understanding of clinical risk assessment and care planning.
- Have good knowledge of medication management and clinical governance.
- Understand the responsibilities associated with delegated clinical healthcare tasks.
- Have excellent written communication and documentation skills.
- Be confident developing and reviewing clinical policies and procedures.
- Be able to work independently during the initial service-development phase.
- Demonstrate a strong commitment to safe, person-centred and high-quality care.
Desirable Experience
It would be advantageous to have:
- Previous experience within a CQC-regulated domiciliary or complex care service.
- Knowledge or experience of the Treatment of Disease, Disorder or Injury (TDDI) regulated activity.
- Experience supporting a service through CQC registration or an extension/variation of registration.
- Experience training and assessing healthcare or support workers in clinical competencies.
- Experience managing or clinically overseeing complex community care packages.
- Previous leadership, clinical lead or nurse assessor experience.
Working Arrangements
During the CQC registration and setup period, the role will initially be for 10 hours per week at £40 per hour, equivalent to £400 per week.
The initial service-development and administrative work can predominantly be undertaken remotely, with occasional attendance at the office/service location where required.
Following successful CQC registration and the commencement of complex care packages, the organisation intends to review the position with the successful candidate, with the opportunity to discuss increased hours and progression into a full-time role.
This is an excellent opportunity for an experienced complex care nurse who would like to be involved in developing a new clinical service from the outset and play a key role in shaping its clinical standards, governance and future delivery.
Pay: £25,420.00-£42,069.19 per year
