The Enhanced Care Home Team are looking for an experienced Healthcare Assistant to join the General Practice team to meet the needs of their patients by providing quality evidence based care. You will play an integral role in planning and organising and delivery of clinical care in line with service and patient needs.
After undertaking additional training, you will support in the promotion of vaccinations and routine immunisations, venepuncture, and other relevant clinical tasks as required by the practice, and in line with your enhanced training, development and competence
- Health Care Assistants are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to this role. Working under supervision of the teams clinical leads and the wider multi professional team, you will play an integral role in planning, organising and delivery of clinical care in line with service and patient needs.
- Work as part of the Enhanced Care Home Team to provide and monitor care, under direct or indirect supervision. Develop relationships across Primary, secondary, and social Care services to support integration of the role across health and social care economy. Act as a link between physical health and mental health to provide a holistic approach for patients.

- Supporting personalised care planning and support plans.
- Enhance own performance through continuous professional development. Attending team meetings, supervision sessions and training courses.
