Job summary
Are you interested in working as a Health Care Support Worker within the Community Rehabilitation Unit ? If so we are looking for someone who can work as an integrated member of the Community Rehabilitation Unit Nursing Team, providing nursing care for patients in the unit under the direction and supervision of the Unit Team Leader. We require a HCSW who can promote a positive and professional image of community nursing within the unit, working with others to support quality of care and ensure the person is at the heart of everything we do. Additionally to undertake delegated administration activities including demonstrating an ability to input into IT systems as required.
The post holder will communicate on a regular basis with the patients, their relatives and colleagues in a caring, compassionate and professional manner using a trauma informed approach.
Provide support to the nurse in charge or the person responsible for the clinical care of the patient.
Exchange factual information with patients using tact, reassurance, empathy, compassion and acknowledging barriers to understanding.
Promote and develop good relationships with patients, staff and visitors.
Recognise potential deterioration and appropriately escalate, working to maintain safety until assistance arrives.

Main duties, tasks & skills required
If you are new to Health Care we will place you on a HCSW induction programme to ensure that you are fully trained and supported to perform the role. The programme will last for up to 6 months and during this period of training you will be paid a salary of £25272. On completion of your training period your salary will rise to £25760 as per the advert.
You will provide compassionate, patient-centred care under the supervision of Registered Nurses and Healthcare Professionals, implementing agreed care plans and supporting patients with daily living activities. Responsibilities include undertaking delegated clinical duties, recording observations, monitoring patient wellbeing, maintaining accurate records, and assisting with personal care, mobility, nutrition, and hydration.
Build positive therapeutic relationships with patients, relatives, and carers, supporting participation in meaningful activities and therapeutic interventions. Safely use manual handling equipment and contribute to delivering high-quality, safe, and effective care as part of a multidisciplinary team.
