Job Summary
We are seeking a proactive and highly organised Domiciliary Care Coordinator to manage the delivery of high-quality home care services. This role is central to our operations, focusing on precise rota planning, ensuring stringent compliance, and conducting thorough service user assessments. You will act as the primary link between our clients and care teams, ensuring that personalised support is delivered safely and efficiently in the community.
Duties
- Service User Assessments: Conduct initial and ongoing assessments of clients in their own homes, developing and updating person-centred care plans that reflect their evolving needs.
- Rota Planning: Design and manage complex staff schedules, ensuring optimal coverage, continuity of care, and travel time efficiency for the field team.
- Compliance & Quality: Monitor and maintain full compliance with CQC standards and health and safety regulations, ensuring all documentation is accurate and audit-ready.
- Staff Supervision: Lead and support a team of home carers, providing guidance, and fostering a culture of excellence.
- Stakeholder Liaison: Act as the key point of contact for families, GPs, and social services to ensure a joined-up approach to client wellbeing.
- IT Management: Utilise care management software for real-time scheduling, digital logging, and reporting.
- Emergency Response: Provide leadership and problem-solving during scheduling conflicts or care emergencies, ensuring service continuity at all times.
Experience
- Sector Expertise: Proven experience working within a domiciliary care setting is essential.
- Coordination Skills: Strong background in rota management and resource allocation within a fast-paced environment.
- Assessment Proficiency: Experience in conducting risk assessments and creating detailed care plans for vulnerable adults.
- Tech Savvy: Competent in using digital care planning and scheduling systems.
- Communication: Excellent interpersonal skills, with the ability to handle sensitive conversations with empathy and professionalism.
- Requirements: A valid driving licence and access to a vehicle is essential for conducting home visits.
