About AMS Case Management and the Role

Case management is the management of a multi-disciplined team of clinical specialists to assess, plan, and monitor the needs of an individual and the services they require to enable the individual to meet their rehabilitation goals, returning them as close as possible to their life before their accident.
Case managers are healthcare professionals (Registered Nurses, Social Workers, Speech and Language Therapists, Physiotherapists, Occupational Therapists). The role of the case manager is to work alongside the client to identify areas of need and agree with the client on an action plan to meet their rehabilitation goals. The case manager will work with the client to set SMART goals - Specific, Measurable, Achievable, Realistic and Time bound. In partnership with the client, the case manager will identify appropriate therapists and professionals to work with the client to meet, and hopefully exceed, the client's rehabilitation goals.

Pay is negotiable depending on experience, plus company pension, mileage, and client expenses.
Main Responsibilities
- To assess the client’s needs and develop appropriate individual case management plans.
- To produce and implement proposals and recommendations for intervention which also provide costs to instructing parties, in order to gain agreement to progress a structured and timely client rehabilitation plan.
- To conduct risk assessments and, where appropriate, produce a risk management plan to demonstrate a minimum level of risk to the client and to all people who interact with, and work with the client.
- To develop and prioritise client’s SMART goal plans to ensure appropriate treatment / action is timetabled.
- To research, identify, and engage relevant specialists / therapists and monitor progress to ensure the quality and effectiveness of the rehabilitation plan and goals.
- To manage and support the client’s engagement in day-to-day activities to ensure the rehabilitation plan and goals are met.
- To liaise with all concerned parties, deliver training in brain injury where appropriate, and review progress against rehabilitation goals, to ensure timely progress.
- To monitor actual costs against proposed costs to ensure work is within budget.
- To provide and submit accurate and timely progress summaries and case notes to enable timely billing.
Qualifications and Experience
Essential:
- Current registration with relevant professional body
- Minimum four years’ post qualification experience, preferably with demonstrated experience in brain injury, spinal injury, physical disabilities or orthopaedics (see additional info for those with less than four years’ experience)
- Full clean driving licence and access to a car
- Willingness to travel across the UK as required for individual cases
- Acceptable standard of home office
- Clean DBS check at enhanced level
Desirable:
- Previous case management experience
- Experience working within community, hospital, or private rehabilitation settings
Additional Info
Less than four years post qualification experience
If you have less than four years’ post qualification experience, or less than four years’ experience as a case manager, the position would be case manager in training. The salary for this would be less than advertised and would be dependent on your experience.

Due to the nature of this work, successful candidates will be subject to an enhanced Disclosure and Barring Service check.
Appointment to the post is subject to satisfactory references, registration checks, proof of holding a full UK driving licence and have been driving for at least 12 months.
Pay: £40,000.00-£50,000.00 per year
