Hours: 37.5 hours per week (Full-time) + shared on-call duties. 9am to 5pm, Monday to Friday with one weekend for staff meeting and On Call.
Logic Care Solutions Limited is seeking an efficient, empathetic Domiciliary Care Coordinator to join our team in Medway, Kent. You will play a crucial part in ensuring our clients receive high-quality, seamless care in their own homes.
What You Will Do
- Create, manage, and optimize daily and weekly staff rotas.
- Match appropriate care workers to clients based on skills and location.
- Act as the primary contact for clients, families, and care teams.
- Monitor digital care logs to ensure zero missed or late visits.
- Responsible for building care runs that are cost effective and efficient
- Liaise with and work closely with the recruitment team to ensure adequate capacity
- Monitor community care workers and carry out supervisions and welfare checks
- Liaise with other professionals involved in the care of our clients
- Participate in a shared, on-call rota for emergencies.
What We Are Looking For
- Minimum 3 years of experience as a care worker or coordinator. Domiciliary care experience preferred.
- NVQ / QCF Level 3 in Health and Social Care (or working towards it).
- Strong IT skills with experience using care rostering software.
- Excellent problem-solving abilities under pressure.
- Pleasant, helpful & concise telephone manner with customer focus.
- Excellent organisational skills and the ability to prioritise in a fast paced environment.
- Patient and empathetic.
- Flexibility to cope with changing demands and priorities.
- Calm under pressure.
- Full UK driving licence with access to your own vehicle.
