It's a rare opportunity to join our friendly Community Learning Disability Team in North Tyneside. Our team is a large well established and supportive multi disciplinary team offering holistic health care to children and adults who have a learning disability living in North Tyneside.
The Ideal candidate will have an understanding of National and Local Policies relating to the management of people with learning disabilities.

The post holder will be welcomed as a member as a member of the friendly, community learning disabilities team in Northumbria Health Care Trust offering regular team meetings, robust support and supervision, and a commitment to continued professional development.
As an integral part of North Tyneside Community Learning Disability Team, the post holder will be solely responsible for a personal caseload including assessment, planning, implementing and evaluation of care.
This post is based in our Primary Care Workstream and the post holders primary role will be promoting health inequalities and supporting patients to access primary care. the post holder will support the dementia pathway and help develop and deliver training to a variety of people supporting the health needs of people who have a learning disability.
They will be passionate about working in collaboration with service users, their family, carers and professional colleagues and will support patients and the wider workstreams within CLDT to enhance health and wellbeing, to prevent or reduce behaviour that challenges and improve mental health outcomes.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres.

To provide a high quality community nursing service to adults and children who have a learning disability within the North Tyneside area.
To work across Primary and secondary healthcare services to ensure equality of access for those with learning disabilities.
To work with other agencies and services (statutory and non statutory) to ensure equality of access for those with a learning disability and facilitate effective communication between clients and other agencies.
To provide community nursing support to adults and children who have behavioural/mental health problems. Support the design and implement Positive Behaviour Support Plans to provide intensive, proactive support and crisis intervention to patients, carers and care providers to prevent/ reduce admission to in-patient units and to maintain community placements.
