About AFL Plus:
Akela Future Living Plus CIC (AFL Plus) provides tailored supported accommodation and Support services for care-experienced individuals and vulnerable adults aged 18 and over. Our mission is to empower individuals to achieve independence, build resilience, and improve their life prospects through dedicated support and guidance.
Job Purpose:
The Support Worker will play a critical role in delivering high-quality, person-centred support to individuals residing in our supported accommodation and in the wider community. You will assist tenants in developing life skills, accessing education, training or employment opportunities, and improving their overall wellbeing, with the aim of fostering independence and social inclusion.
Key Responsibilities:
- Provide flexible, strengths-based support tailored to the needs of each individual.
- Conduct regular key working sessions and support visits in tenants' homes and community settings.
- Assist individuals in developing and maintaining independent living skills, such as budgeting, cooking, cleaning, and tenancy management.
- Support individuals in accessing external services including education, training, employment, healthcare, and specialist support services.
- Promote social inclusion by supporting individuals to engage in community activities and reduce isolation.
- Maintain accurate and up-to-date support plans, risk assessments, and case notes in line with AFL Plus policies.
- Build trusting and professional relationships with tenants, external professionals, families, and stakeholders.
- Advocate for individuals’ rights and needs when liaising with external agencies such as housing officers, social workers, and healthcare providers.
- Identify and escalate safeguarding concerns in line with AFL Plus safeguarding policies and procedures.
- Contribute to the continuous improvement of AFL Plus services by providing feedback, insights, and suggestions.
Person Specification:
Essential:
- Experience of working with vulnerable adults, care leavers, or individuals with complex needs.
- Knowledge of housing-related support, benefits systems, and pathways to independence.
- Strong interpersonal skills with the ability to build rapport and maintain professional boundaries.
- Understanding of safeguarding procedures and a commitment to promoting the safety of individuals.
- Ability to work independently and manage a flexible caseload.
- Full UK driving licence and access to a vehicle.
Desirable:
- Relevant qualifications in Social Care, Youth Work, or Community Development.
- Experience in delivering outcome-based support.
- Knowledge of local community resources and support networks.
