Job Purpose
To provide high-quality, person-centred support that enables individuals to live independently, achieve their goals, and participate fully in their communities.
You will support individuals with a range of needs, including:
- Learning disabilities
- Autism / Asperger’s
- Mental health needs
- Physical disabilities
- Complex health needs or behaviours
- Communication or sensory impairments
Key Responsibilities
Supporting Individuals
- Support people to live independently and make their own choices
- Enable individuals to achieve personal goals and outcomes
- Promote inclusion in the community based on individual interests
- Build and maintain positive relationships with families, carers, and professionals
- Work across settings including supported living, family homes, and community environments
Person-Centred Practice
- Deliver support tailored to individual needs, preferences, and communication styles
- Involve individuals in all decisions about their care
- Advocate on behalf of individuals where appropriate
- Promote equality, diversity, and positive representation
Care & Daily Support
- Provide personal care and physical support where required
- Administer medication safely (following training and policy)
- Support health, wellbeing, and safety
- Respond appropriately to behaviours that may challenge
- Ensure a safe home and community environment
Planning & Records
- Contribute to support plans, risk assessments, and reviews
- Maintain accurate and up-to-date records
- Support individuals to lead their own care planning
- Support with financial tasks and maintain accurate financial records where required
- Contribute to and follow risk assessments and risk management plans
- Maintain accurate records including Health Action Plans and individual support documentation
- Promote independence by ensuring support is proportionate and not excessive
- Work in line with agreed shift patterns including sleep-ins and waking night responsibilities where required
Team & Professional Responsibilities
- Work collaboratively as part of a team
- Follow all policies and procedures (including safeguarding, health & safety, and confidentiality)
- Engage in training, supervision, and ongoing development
