The Population Health Management (PHM) team is a system-supported programme working across Lewisham in collaboration with local health and care partners. The programme uses data and analytics to understand the health and care needs of the population, identify inequalities and unwarranted variation, and support improvements in health outcomes and service delivery.
The team is building on a new ly developed Snowflake data environment, currently in the early stages of release , bringing together integrated health and care datasets into a unified analytical environment. This will support a more comprehensive understanding of population health and enable the development of new analytical tools, metrics and approaches.
The Population Health Data Analyst will contribute to the development and use of this environment, including scripting, querying, developing metrics and supporting the integration and analysis of health and care data.
Candidates identified as being at risk of redundancy within South East London ICB or Lewisham and Greenwich NHS Trust will be given priority consideration in accordance with the applicable redeployment processes before any appointment is made from the wider applicant pool.
The analytics workstream within the Population Health Programme supports clinicians, commissioners and programme managers to use data to understand population needs, identify opportunities for improvement and inform decision-making.
Key skills & experience:
Strong experience of working with health and/or social care data across multiple sources.
Experience of analysing and interpreting complex health and care data.
Strong analytical and programming skills, particularly SQL and Snowflake, with experience of Power BI.
Experience of developing metrics, queries and analytical products.
Experience of working with integrated health and care datasets.
Experience of supporting population health, neighbourhood health, proactive case finding or service improvement would be advantageous .
Understanding of health inequalities and the importance of using data to identify and address inequalities.
Excellent communication skills, with experience of communicating complex information to clinicians, commissioners, programme managers and other non-technical audiences.
Understanding of information governance, data quality and the importance of using the right data appropriately.
Ability to work autonomously, prioritise workloads and manage competing demands.
A data-led and creative approach to problem solving, with the ability to develop practical analytical solutions to complex questions.

Key Result Areas & Performance
Work with the Principal Population Health Analyst and Portfolio Manager to deliver the Population Health Analytics work programme.
Provide analytical support to Integrated Neighbourhood Teams, initially focusing on long-term conditions, frailty and pharmacy.
Develop and maintain analytical solutions within the Snowflake environment, including SQL queries, scripting and population health metrics.
Support the development of integrated health and care datasets and reusable analytical products.
Develop and scope enhanced risk tools to support proactive case finding and preventative care.
Support primary care practices with population monitoring and data-led improvement.
Provide analytical support to MDM meetings and other clinical and care teams.
Support the design, monitoring and evaluation of data-led interventions.
Undertake analytical work for acute trust improvement projects and other service transformation programmes.
Support basic costing and ROI analysis where required .
Ensure health inequalities are considered within population health analysis and outputs.
Ensure appropriate data security, information governance and data quality standards are maintained .
Workforce
Work collaboratively with analysts, data engineers, clinical colleagues and programme teams.
Provide peer support and guidance on analytical approaches, data interpretation and population health products.
Share knowledge and contribute to the development of analytical capability across the programme.
Communications and Relationships
The post holder will work closely with clinicians, commissioners, programme managers, primary care teams, Integrated Neighbourhood Teams, acute trust colleagues, analysts, data engineers and the AI Data Centre at GSTT.
The post holder will:
Communicate complex analytical information clearly to technical and non-technical audiences.
Work with stakeholders to understand their key questions and translate these into appropriate analytical approaches.
Explain data quality, limitations and the importance of using the right data for the right purpose.
