Salary: £44,000 – £47,000 WTE, dependent on experience

Dashwood Primary Care Network has an exciting opportunity for a Clinical Pharmacist to join our established and growing team.
We are seeking an enthusiastic individual who is passionate about delivering high-quality patient care and keen to become an integral member of a progressive clinical pharmacy team.
This is a patient-facing role offering excellent opportunities to utilise and develop your clinical skills to improve the health outcomes of the local community. Responsibilities include:
- Managing long-term conditions
- Supporting hospital discharge prescribing
- Providing advice to patients on complex medication regimes (polypharmacy)
- Contributing to clinical audits to improve patient care
- Supporting the development of the clinical pharmacist role within GP practices
You will receive support, mentorship, and training from a Senior Clinical Pharmacist, along with dedicated GP supervision time.
Requirements
- GPhC registration
- MPharm degree (or equivalent)
- Successful completion of the CPPE Primary Care Pharmacy Education Pathway
- Experience in Primary Care or within a Primary Care Network

Key Responsibilities
- Work as part of a multidisciplinary team in a patient-facing role to assess and treat patients using expert knowledge of medicines
- Manage chronic diseases and conduct structured medication reviews for patients with complex polypharmacy
- Develop and implement medicines management plans in line with local and national guidance
- Maximise cost-effective prescribing and improve the quality of patient care
- Provide specialist advice on medicines, addressing public health and social care needs while helping to tackle health inequalities
- Build strong relationships with pharmacy professionals across PCNs and the wider healthcare system
- Contribute to shared care protocols, clinical research, and liaison with specialist pharmacists, community pharmacies, and anticoagulation services
