About the Role
As an Associate Clinical Case Manager, you will manage your own caseload while benefiting from the support, expertise and professional reputation of Westcountry Case Management.
You will work collaboratively with clients, families, deputies, solicitors and treating professionals to assess needs, coordinate rehabilitation programmes and ensure clients have access to the services and support required to achieve their rehabilitation goals.
