The Health Care Group (THCG) Ltd is seeking an experienced and organised Senior Care Coordinator to join our established domiciliary care service in Sutton.
This is a key office-based role supporting the effective day-to-day coordination of care services across Sutton and the surrounding areas. The successful candidate must have a minimum of two years’ experience working as a Care Coordinator within a domiciliary care setting and must be confident managing care rotas, responding to changes, communicating with care staff and ensuring continuity of care for service users.
You will work closely with the Registered Manager, care staff, service users, families, social workers and other healthcare professionals to ensure care is delivered safely, efficiently and in line with CQC requirements.
Key Responsibilities
- Coordinate and maintain care rotas, ensuring all care visits are allocated safely and effectively.
- Respond promptly to staff sickness, absence, annual leave and changes to service-user requirements.
- Ensure continuity of care by matching care staff appropriately to service users.
- Monitor care visits and address late, missed or incomplete calls.
- Communicate regularly with service users, families, care staff and healthcare professionals.
- Support the development, review and updating of person-centred care plans and risk assessments.
- Maintain accurate and up-to-date records on the electronic care management system.
- Support medication audits, care-record audits, spot checks and staff supervisions.
- Assist with the recruitment, induction and ongoing support of care staff.
- Provide guidance and support to care assistants regarding rotas, client needs and operational concerns.
- Record and respond appropriately to complaints, incidents, safeguarding concerns and changes in service-user needs.
- Liaise with local authorities, social workers, district nurses and other professionals where required.
- Support the Registered Manager in maintaining compliance with CQC regulations and company policies.
- Participate in the out-of-hours on-call rota when required.
- Assist with service mobilisation, new care packages and initial assessments where appropriate.
- Ensure confidential information is handled in accordance with UK GDPR and company procedures.
Essential Requirements
- A minimum of two years’ experience working as a Care Coordinator within a domiciliary care service.
- Strong experience coordinating care rotas and managing changes at short notice.
- Good understanding of CQC regulations and the requirements of a regulated home-care service.
- Experience supporting a busy care service with multiple service users and care staff.
- Excellent communication and customer-service skills.
- Strong organisational and problem-solving abilities.
- Ability to remain calm and professional when dealing with urgent or challenging situations.
- Good IT skills, including Microsoft Word, Excel, Outlook and electronic care-management systems.
- Ability to maintain accurate records and manage confidential information appropriately.
- Full UK driving licence and access to a vehicle.
- Applicants must already have the unrestricted right to work in the UK.

Desirable Requirements
- Level 3 Diploma in Adult Care, Health and Social Care or an equivalent qualification.
- Experience using Birdie or a similar electronic care-management and rostering system.
- Experience completing care plans, risk assessments, medication audits, supervisions and spot checks.
- Previous experience as a Senior Care Coordinator, Field Care Supervisor or Deputy Manager.
- Knowledge of local authority commissioned care services within Sutton or surrounding London boroughs.
