Wellbeing and Activities Co-ordinator

Reports to: Home Manager
Accountable to: Home Manager and Directors

About the role

We are looking for a warm, creative and organised Wellbeing and Activities Co-ordinator to create meaningful experiences for our service users.

You will plan and deliver an engaging seven-day programme of activities, events and outings that reflects each person’s interests, abilities, culture, history and choices. Working closely with the nursing and care teams, families and the local community, you will help make every day enjoyable, inclusive and person-centred.

Key responsibilities

Plan, coordinate and deliver a varied seven-day programme of indoor, outdoor, group and one-to-one activities.
Get to know each service user’s life history, interests, preferences, abilities and wellbeing needs.
Create individualised activity and wellbeing plans, reviewing and adapting them regularly.
Arrange meaningful activities such as arts and crafts, music, reminiscence, games, exercise, celebrations, gardening, themed events and community visits.
Organise safe outings, trips and holidays, ensuring appropriate support and risk assessments are in place.
Encourage service users to maintain independence, make choices and take part at their own pace.
Ensure activities are inclusive and culturally sensitive, respecting differences in culture, faith, race, gender, sexuality and personal identity.
Build positive relationships with relatives, friends, volunteers, local community groups and other Apex Homes.
Organise entertainment, fundraising events and opportunities for service users to socialise with the wider community.
Work closely with care and nursing colleagues to identify changes in wellbeing, mobility, mood or support needs.
Maintain accurate, timely records of activities, attendance, outcomes and individual wellbeing.
Raise safeguarding, welfare, accident, incident or health and safety concerns immediately with the appropriate senior staff.
Support a caring, respectful culture that promotes dignity, privacy, rights, choice and independence.
Assist with the day-to-day running of the home when required, under the direction of the nurse in charge and Home Manager.
Attend mandatory training, fire drills, meetings and role-relevant development opportunities.

What we are looking for

A caring, enthusiastic and imaginative approach.
Experience of planning activities or supporting people in a care, dementia, learning disability, community or wellbeing setting.
Strong communication and relationship-building skills.
Ability to motivate and encourage people with different needs and abilities.
Good organisational skills and confidence managing an activity calendar.
A person-centred approach, with genuine respect for individual choice and dignity.
Awareness of safeguarding, confidentiality, equality, diversity and inclusion.
Ability to keep clear, accurate records.
A driving licence is desirable for supporting outings and use of company vehicles.
