Job Overview
We are seeking a dedicated and highly skilled Advanced Nurse Practitioner to join our dynamic healthcare team. · The post holder will be an experienced healthcare practitioner with experience in acute and urgent care. Ideally, they will have completed an MSc in Advanced Practice and must be independent prescribing. The post holder must be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced clinical practitioners are expected to work across the four pillars of advanced practice; clinical, leadership, research and education.
· The post holder will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.
· The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Key Responsibility:
- Independently assess, diagnose, treat, prescribe for, and manage patients with acute, chronic, complex, undifferentiated, and undiagnosed conditions, including undertaking home visits where appropriate.
- Provide same-day urgent care, triage, clinical decision-making, and definitive treatment within scope of practice.
- Undertake comprehensive history taking, physical and mental health assessments, risk assessments, clinical examinations, diagnosis, treatment planning, implementation, review, and follow-up of care.
- Assess, diagnose, and manage long-term conditions, minor illness and injury.
- Act as an Independent V300 Prescriber, prescribing safely and effectively in accordance with national and local guidelines, policies, and evidence-based practice.
- Request, perform (where competent), interpret, and act upon diagnostic investigations, including pathology, ECGs, and imaging.
- Refer appropriately to GPs, clinical leads, secondary care, community services, mental health teams, and other specialist services, seeking supervision or second opinions when required.
- Develop personalised care plans and actively involve patients and carers in decision-making, promoting self-management, health education, prevention, and wellbeing.
- Build effective relationships and work collaboratively with multidisciplinary teams across primary, secondary, community, statutory, and voluntary services to ensure coordinated, seamless patient care and avoid unnecessary hospital admissions.
- Provide specialist clinical advice and support to patients, carers, and healthcare professionals on the assessment and management of physical and mental health conditions.
- Deliver clinical leadership, mentorship, supervision, and support to less experienced staff, delegating responsibilities appropriately within the team.
- Maintain accurate, contemporaneous, and legally compliant clinical records and documentation.
- Participate in multidisciplinary team meetings, clinical governance, audits, quality improvement initiatives, research, education, and service development activities.
- Contribute to patient safety through the investigation of incidents, complaints, significant events, and implementation of learning and service improvements.
- Maintain professional competence through continuing professional development, mandatory training, reflective practice, and adherence to professional, regulatory, and organisational standards.
- Support the achievement of practice objectives, quality standards, enhanced services, contractual requirements, and continuous improvement initiatives.
Essential
- NMC Registered Nurse with current registration.
- Master's degree in Advanced Clinical Practice
- Independent/Supplementary Prescriber qualification (V300).
- Advanced Clinical Practice qualification or equivalent.
- Evidence of ongoing Continuing Professional Development (CPD).
Desirable
- Experience working in General Practice or Primary Care Networks.
- Experience in Community Mental Health Services.
- Experience supervising junior staff or students.
- Experience leading quality improvement projects.
Pay: £35,360.00-£66,300.00 per year
