Full job description

Primary Care Clinical Pharmacist - Market Rasen Surgery

We are seeking an enthusiastic and forward-thinking clinical pharmacist to join Market Rasen Surgery, Lincolnshire. We are looking for a pharmacist to develop and manage all aspects of medicines across the practice network. Working with highly skilled multi-disciplinary teams, we currently have 2 pharmacists and 1 pharmacy technicians. You will work in a patient-centred role based in the surgeries along with some hybrid working. This is an evolving role with huge scope for development, we will support you with your independent prescribing course if required. Full details can be found in the Job Description and Person Specification attached.

The successful applicant will be a pharmacist who acts within their professional boundaries, supporting and working alongside a multi-disciplinary team in a patient facing role. Please note: this is a hybrid role and you will be required to attend the practice for more than 50% of the time, during the working week, which is essential for this role.

The type of duties involved will be:

Telephone & some face to face consultations and reviews with patients
Managing repeat prescriptions
Responding to patient askmygp queries related to medication.
Long term condition reviews - training will be provided for this if necessary
Synchronise medication
Review urgent prescriptions
Liaise with hospital pharmacists
Provide regular support and feedback to practice on prescribing action plans.
Updating patient records
Ensuring the practice is compliant with CQC medicine-related standards
Undertaking risk assessments and ensuring compliance with legislation
Audits

All practices use SystmOne clinical IT system.

Job Type: Permanent

Working Pattern: part time (30 hours)

Salary: Competitive rate dependent on experience. The salary advertised is for full time equivalent.
