We are looking for a warm, thoughtful and organised Dementia Navigator to join our supportive team in Bristol working 21 hours per week.
This is a community-based role, so you'll regularly travel across Bristol area to meet people in their homes, community venues and partner locations. As such, the ability to travel independently across the area is essential.
As a Dementia Navigator, you'll take a person-centred approach to help people understand their diagnosis, navigate health and social care services, access local support and feel more in control of their lives. You'll build trusted relationships with individuals, families and professionals, ensuring people receive the right support at the right time.
You will be part of a pro-active, caring and supportive local team who work in a fully integrated partnership with NHS colleagues, supporting people from point of diagnosis through to end of life.
About you
We're looking for someone who is passionate about supporting people and making a real difference.
You'll have:
- An understanding of the needs of people living with dementia and their carers.
- Experience of working with health, social care, voluntary or community organisations.
- Experience of assessing people's needs and providing person-centred support.
- Communication and relationship-building skills, with the ability to adapt your approach to different audiences.
- Organisational skills and the ability to manage your own workload effectively.
- Experience of maintaining accurate records and using IT systems confidently, including Microsoft Office and databases.
- An understanding of confidentiality, safeguarding and professional boundaries.
- A commitment to equality, diversity and inclusion, ensuring everyone can access support that meets their individual needs.

Key Responsibilities:
- Providing tailored information, advice and emotional support to people affected by dementia.
- Managing your own caseload and developing personalised support plans that respond to changing needs.
- Working closely with GPs, memory clinics, health and social care professionals and voluntary sector partners to ensure coordinated support.
- Signposting and referring people to appropriate local and national services.
- Keeping accurate and confidential case records using our internal systems.
- Building relationships with community organisations to improve access to services, particularly for underrepresented and marginalised communities.
- Identifying opportunities for people affected by dementia to influence services, take part in research or become involved in the work of Alzheimer's Society.
- Promoting safeguarding, inclusion and best practice in everything you do.
