As part of a multidisciplinary team, the post holder will be responsible for assessing care and support needs, developing person-centred care plans and risk assessments, and implementing, monitoring and evaluating these to ensure the delivery of safe, high-quality care and positive outcomes for people we support.

The post holder is expected to carry out all aspects of care and support with a high degree of autonomy, while working within Company policies and professional standards. They may also be required to demonstrate procedures, mentor and supervise SSSC-registered care staff.

The post holder will lead, coordinate and supervise the work of their designated team, promoting best practice, person-centred approaches and positive outcomes for people we support across a diverse range of needs and circumstances.

The post holder will also support the Service Manager and Deputy Service Manager with the day-to-day running of the service, undertaking delegated operational and administrative tasks as required to ensure the smooth, safe and effective delivery of care.

Main Responsibilities

Lead, supervise and support a team of Support Workers, providing induction, mentoring, guidance and ongoing development
Deliver and coordinate high-quality, person-centred care in line with Company policies, procedures and values, promoting independence, dignity, choice and positive outcomes for people we support
Assess care and support needs and develop, implement and review person-centred care plans and risk assessments, ensuring they remain current and outcome-focused
Support the Service Manager and Deputy Service Manager with the day-to-day operational running of the service, carrying out delegated duties and administrative tasks as required
Manage your designated team and rota, ensuring effective staffing levels, mandatory training compliance and appropriate allocation of resources
Maintain accurate care records and communicate effectively with people we support, families, professionals and the wider multidisciplinary team
Ensure compliance with Company policies, regulatory standards, safeguarding, health and safety, and all relevant legislation
Support staff performance through supervision, coaching, wellbeing conversations and constructive feedback
Participate in audits, quality assurance and continuous improvement activities
Act as a positive role model, demonstrating the Company’s values and promoting a supportive, inclusive team culture

Qualifications and Experience

Full UK driving licence
Valid SSSC Registration (or willingness to obtain)
SVQ Level 3 in Health Social Care (or willingness to work towards)
Willingness to participate in the on-call rota
Previous experience in a health or social care setting, with supervisory or leadership experience desirable
Experience of assessing care needs, developing care plans and risk assessments, and supporting adults with a range of support needs
Strong communication, organisational and leadership skills
Basic IT skills and evidence of ongoing professional development
Flexibility to work a varied shift pattern, including 12-hour day or night shifts

*This position requires a PVG and two references*
