Job Introduction
As a Band 6 Physiotherapist, you will join our valued Rapid Response and Intermediate Care Team based at Sheppey Community Hospital. This is a dynamic role where you will deliver urgent, short-term rehabilitation and assessment to patients in their own homes and community settings. You will play a key role in preventing hospital admissions and supporting early discharge, helping patients remain safe and independent in their own environment. This is a fast paced and rewarding role, ideal for someone who enjoys variety, autonomy and making a real difference within the community. You will be part of a supportive and experienced multidisciplinary team, working closely with Occupational Therapists, Nurses and support colleagues to deliver high quality, patient centred care.

The service operates 7 days a week between 8am and 8pm, so flexibility to work weekends and bank holidays is required.
In this role, you will deliver specialist physiotherapy assessment and intervention to patients with a wide range of acute and long term conditions. You will work within a rapid response model, carrying out urgent assessments, triaging referrals and supporting timely, safe care in the community. You will complete holistic assessments, develop and review care plans, and deliver targeted rehabilitation to support recovery and independence. You will also play an important role in preventing unnecessary hospital admissions and facilitating early supported discharge. Working closely with the wider multidisciplinary team, you will contribute to coordinated care delivery and actively support service development. You will also supervise junior colleagues, support workers and students, helping to build capability across the team. Key responsibilities include:

Completing complex, holistic patient assessments in community settings

Triaging referrals from a range of sources

Delivering and reviewing personalised rehabilitation programmes

Delegating and supporting rehabilitation delivery by support workers

Liaising with families, carers and partner services

Supporting clinical learning through supervision, training and student placements

Contributing to team meetings, service development and quality improvement
Essential
-
Degree in Physiotherapy
-
HCPC registration
-
Experience working across a range of clinical areas
-
Strong assessment and clinical reasoning skills
-
Ability to work independently and manage a varied caseload

Full UK driving licence and access to a car
-
Excellent communication and organisational skills
-
Confidence using IT systems, including Microsoft Office
Desirable

Experience within community, rapid response or intermediate care services

Experience supervising staff or students

Interest in urgent care, admission avoidance and rehabilitation in the community

Experience working within a multidisciplinary team
