North Sedgemoor PCN is seeking an enthusiastic and committed Salaried GP to join our friendly, progressive team working across three practices within our PCN, Brent Area Medical Centre, Axbridge Medical Centre and Cheddar Medical Centre for 16 sessions per week.

This is an exciting opportunity to become part of a well-established, collaborative group of practices serving a diverse rural and semi-rural population. We pride ourselves on highquality patient care, strong multidisciplinary teamwork, and a culture that supports clinicians to thrive.

Main duties of the job:

Provide medical services for the practice population to a high standard, including day-to-day activities such as surgery consultations, telephone advice, telephone consultations and home visits and appropriate administration to ensure accurate records, patient continuity and quality of care.
Comply with the requirements of the practices General Medical Services/Personal Medical Services (GMS/PMS) (17c) contract, Enhanced Services contracts, educational/training delivery contracts and private contracts.
Adhere to the principles of good medical practice.

Overview of our organisation:

North Sedgemoor PCN brings together GP practices caring for around 50,000 people across Burnham on Sea, Brent, Highbridge, Cheddar and Axbridge. We are clinically led, collaborative and community focused. Our team includes GPs, Occupational Therapists, Pharmacists, Care Coordinators, Health Coach and an experienced Paramedic who will be a peer and mentor for this role.

We are a learning organisation. You will have a named supervisor, regular clinical forums and protected time for development. We value curiosity, high standards and kindness. Ideas are welcomed and improvement is part of daily work.

This is an exciting time to join us. We are scaling services and modernising how we deliver urgent and planned care in practices, in peoples homes and in care homes. You will be supported by clear pathways, friendly administrators and a welcome team who help new colleagues settle in.

We offer funded training aligned to your scope of practice, access to First Contact Practitioner routes, Masters level modules and Independent Prescribing where appropriate. You will also have NHS pension if eligible, generous annual leave and access to wellbeing resources.

Qualifications:

Essential criteria:

Registered GP on the GMC Specialist Register.

Experience:

Essential criteria:

Previous experience in managing complex long-term conditions, elderly care, or palliative care
Excellent communication, leadership, and decision-making skills in high-uncertainty scenarios.

Desirable criteria

Experience in Proactive Care, creating individualised care plans, focusing on reducing unnecessary hospitalisations and managing palliative needs.
