Full job description

An exciting opportunity has arisen for an enthusiastic and experienced Complex Case Manager to join the Continuing Healthcare (CHC) Team within North West London.

This post is ideal for a Registered Nurse or suitably experienced Allied Health Professional who is passionate about improving outcomes for people with complex health needs.

Working within a specialist multidisciplinary team, you will assess, monitor and review individuals in line with the National Framework for NHS Continuing Healthcare. You will work closely with service users, families, health and social care professionals, provider organisations and local authorities to ensure individuals receive person-centred, safe and effective care that meets their assessed needs.

The role offers an excellent opportunity to develop expertise in Continuing Healthcare, commissioning and complex care assessment while gaining experience across a range of health and social care services. Applications are welcomed from those already working in Continuing Healthcare, as well as candidates looking to broaden their experience beyond traditional clinical settings. Development opportunities may be considered for the right candidate.

Effectively provide quality and commissioning case input and oversight to clients with a learning disability and complex care needs.

Plan, implement and evaluate care plans for clients and their carers using evidence based and person-centred principles

Provide specialist nursing advice and support to clients with a learning disability and complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence based practice

Liaise and work in a joined-up way with the client, their family, relevant health and care professionals, and all other health and social care agencies involved in the care of the person

Contribute to, or lead on (as appropriate), relevant assessments and reviews that support care planning for complex clients with a learning disability

Represent the ICB in complex care panels to provide clinical oversight, risk management, and help inform discussions effecting resource allocation, including making recommendations to commissioners on what care should be commissioned to support that individual to meet their needs.

Cultivate knowledge of local employment, community and universal services that individuals can access to promote their health and wellbeing, as well as knowledge of highly specialized and/ or bespoke interventions

Help quality assure and monitor community and/or inpatient providers the ICB commissions for individuals with a learning disability with complex care need
