Full job description

An exciting opportunity has arisen for a Band 5 Occupational Therapist to join Central London Community Healthcare Trust’s well-established rotation in Barnet, North London .

Rotations include Intermediate Care Services, Unplanned Care (Rapid Response, D2A), Inpatient Rehabilitation, and Stroke Rehabilitation , each lasting six months with start dates in April and October .

Successful applicants will benefit from regular supervision, peer support groups, structured appraisals and a variety of learning and development opportunities to support career growth .

We welcome applications from Occupational Therapists with a BSc or MSc and HCPC registration , excellent organisational and communication skills and experience assessing needs, setting goals and creating treatment plans across diverse healthcare settings .

A UK driving licence and access to a vehicle are desirable. Applicants must be able to work 37.5 hours per week across a 7-day service and travel between locations .

Undertake occupational therapy assessments for a designated caseload, addressing self-care, productivity, and leisure needs .

Develop person-centred intervention plans to maximise independence and reflect clients’ goals .

Work independently across hospital sites, homes, and community settings .

Collaborate with colleagues to deliver individual and group interventions .

Prioritise referrals and manage caseloads within service standards .

Work with clients, families, and professionals to set occupational therapy goals as part of care plans .

Plan and implement graded activities to achieve therapeutic objectives .

Monitor and review treatment to measure progress and adjust interventions .

Conduct home assessments, identifying adaptations or assistive technology to enhance safety and independence .
