Overview
We are seeking a dedicated and skilled GP Practice Paramedic to join our healthcare team. The successful candidate will play a vital role in delivering high-quality patient care within a general practice setting. This position offers an excellent opportunity for experienced paramedics to utilise their clinical skills in a primary care environment, providing essential services and support to patients. The role requires a proactive approach, strong clinical knowledge, and the ability to work effectively within a multidisciplinary team.
Duties
- The post-holder will work as part of the practice clinical team to provide safe, effective and timely assessment, treatment and management of patients, with a particular focus on same-day/urgent presentations, frailty, housebound patients and home visits.
- The role will include assessing patients both in the surgery and in their own homes, undertaking clinical observations and examinations, identifying red flags, making appropriate differential diagnoses, and agreeing management plans within the post-holder’s scope of practice.
- To undertake home visits for housebound, frail, elderly or acutely unwell patients, including assessment of new or worsening symptoms.
- To assess and manage same-day urgent presentations, including minor illness, minor injury, infections, exacerbations of chronic disease, falls, reduced mobility, frailty concerns and deteriorating patients.
- To carry out appropriate clinical assessments, including history taking, physical examination, observations, NEWS2 scoring where appropriate, and clinical decision-making.
- To identify red flags and clinical deterioration, and to escalate to a GP, ambulance service, urgent care, community services or hospital where clinically appropriate.
- To support the management of patients with long-term conditions where relevant to the paramedic’s competence and scope of practice.
- To liaise with GPs, nurses, pharmacists, care homes, community teams, ambulance services, hospital teams and families/carers to support safe patient care.
- To undertake medication-related discussions within scope of practice, recognising when prescribing or medication review requires GP/pharmacist input.
- To document all consultations clearly and accurately in the clinical system, including assessment findings, advice given, safety-netting and follow-up arrangements.
- To support admission avoidance where safe and appropriate, including use of community services, urgent response teams, virtual wards, frailty teams and same-day emergency care pathways.
- To contribute to practice quality improvement, audit, safeguarding, significant event reviews and clinical governance.
- To recognise the limits of their own competence and seek GP or senior clinical support where required.
- To provide clear safety-netting advice to patients and carers, including when to seek urgent or emergency help.
