We are looking for an enthusiastic, patient-focused Clinical Pharmacist to join our friendly and forward-thinking Primary Care Network (PCN) team.
This is an exciting opportunity to play a key role within our multidisciplinary team, helping to improve patient outcomes through expert medicines optimisation, structured medication reviews, prescribing support and long-term condition management.
You'll work closely with GPs, nurses, pharmacy technicians and other healthcare professionals to provide high-quality, evidence-based pharmaceutical care while supporting patients to get the very best from their medicines.
Whether you're already an Independent Prescriber or working towards your qualification, you'll be supported to develop your skills and make a real difference to the health of our local community.
What You'll Be Doing
As our Clinical Pharmacist, you will:
- Deliver structured medication reviews and medicines optimisation clinics.
- Manage patients with complex polypharmacy and long-term conditions.
- Provide expert advice on safe, evidence-based and cost-effective prescribing.
- Undertake face-to-face and telephone consultations.
- Support medication reconciliation following hospital discharge.
- Monitor high-risk medicines and improve patient safety.
- Lead on prescribing quality improvement initiatives and clinical audits.
- Work collaboratively with GPs, nurses, community pharmacists and secondary care colleagues.
- Contribute to Quality and Outcomes Framework (QOF), prescribing incentive schemes and wider quality improvement projects.
- Support education and training for the wider practice team.
- Help shape and develop innovative pharmacy services across the PCN.
About You
We're looking for someone who is:
- Registered with the General Pharmaceutical Council (GPhC).
- Committed to delivering outstanding patient-centred care.
- Experienced in undertaking clinical medication reviews.
- Confident working both independently and as part of a multidisciplinary team.
- Passionate about medicines optimisation and improving prescribing quality.
- An excellent communicator with strong clinical decision-making skills.
- Committed to continuous professional development.
Essential Requirements
- GPhC Registered Pharmacist.
- Minimum two years' post-registration experience.
- Enrolled on, or completed, an approved Primary Care Clinical Pharmacist Training Pathway (or equivalent).
- Hold, or be working towards, an Independent Prescribing qualification.
- Evidence of continuing professional development.
Desirable
- Experience within General Practice or Primary Care Networks.
- EMIS Web experience.
- Postgraduate clinical qualification.
- Membership of the Royal Pharmaceutical Society.
- Specialist clinical expertise in long-term conditions.
