JOB DESCRIPTION
Hours: Full time
Responsible to: Management Partner & ACP Partner (Clinically)
The following list is not exhaustive and may be reviewed in the light of changing circumstances at a personal or organisation level.
Leadership of the Nursing Team:
- The leadership and management of the practice nursing team, consisting of nurses, nursing associates, assistant practitioners and HCAs.
- Overall responsibility of several clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
- Implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
- To act as a role model, demonstrating and promoting the standards expected by the practice and creating the energy and enthusiasm for the service.
- Conduct return to work interviews and annual appraisals, 1-2-1’s with the team.
- Promote teambuilding, teamwork and co-operation to inspire motivation and commitment to meeting practice objectives.
- Awareness of staff wellbeing and support when needed.
- To be key member of the Compass House Leadership Team, providing support and assistance where required to the Management Partner and other members of the Leadership Team. It’s important to be proactive, forward thinking and adaptable.

Clinical Duties:
- Deliver a range of general practice nursing services, including chronic disease management, immunisations, wound care, cervical screening, travel vaccination advice and administration, assisting with minor procedures, venepuncture, ECGs and dressings, and NHS health checks.
- Triage patients and provide health promotion, lifestyle advice, and disease prevention support.
- Making independent clinical decisions alongside or instead of GP.
- Monitoring treatment adherence and recommending changes.
- Mental health - Communicating with key workers, monitoring therapy side effects.
- Ensure accurate documentation of patient interactions using the clinical system.
- Actively participate in safeguarding processes, working with the practice team and external agencies.
- Work within PGD’s and ensure these are kept up to date
Prescribing
- Where the post-holder is an independent non-medical prescriber: To prescribe safe, effective and appropriate medication as defined by current legislative framework.
Infection Prevention and Control (IPC) Lead Responsibilities:
- Lead and manage the surgery’s infection control programme in line with national guidance and CQC requirements.
- Develop, implement, and monitor IPC policies and protocols within the practice.
- Conduct regular audits (e.g. hand hygiene, environmental cleanliness, sharps safety), and ensure actions are followed up.
- Ensure IPC training and updates for all staff are complete, including new starters and annual refreshers.
- Liaise with local IPC teams and Public Health where necessary, especially during outbreaks or incidents.
- Ensure the practice is prepared for and compliant with CQC infection control requirements and reports.
- Maintain and act on an IPC risk register and ensure learning from incidents is embedded.
Clinical Governance and Quality Improvement:
- Participate in clinical audits, patient safety initiatives, and the implementation of NICE or local guidance.
- Monitor and contribute to achievement of QOF and other contractual targets.
- Ensure adherence to evidence-based practice, and support protocol reviews and policy updates.
- Direct responsibility for reviewing and delivering policies
Communication and Team Working:
- Work collaboratively with the multi-disciplinary team, including GPs, administrative staff, pharmacists and physiotherapists.
- Proactive identification and support of carers
- Attend and contribute to clinical and operational meetings.
- Attend nurse and all staff meetings
- Build positive working relationships internally and externally to promote integrated care.
Risk assessment
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
Professional expectations:
- Work within NMC Code of Conduct and maintain professional registration.
- Lead by example in promoting infections control, patient safety and professional standards
- Keep practice up to date by actively participating in professional development and completing mandatory training.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

Health & safety:
The post-holder will implement and lead on a full range of promotion and management of their own and others’ health and safety and infection control as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include (but will not be limited to):
- Using personal security systems within the workplace according to practice guidelines.
- Awareness of national standards of infection control and cleanliness; regulatory / contractual / professional requirements; and good practice guidelines.
- Responsible for the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.
- Management and maintenance of Personal Protective Equipment (PPE) for the practice, including provision, ordering, availability and ongoing correct usage by staff.
- Responsible for hand hygiene across the practice.
- Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice.
- Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed, and weaknesses / training needs are identified, escalating issues as appropriate.
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process.
- Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes.
- Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand-cleansing facilities, wipes etc. are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
- Safe management of sharps procedures including training, use, storage and disposal.
- Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile way, safe and free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management.
- Actively identifying, reporting and correcting health and safety hazards and infection hazards immediately when recognised.
- Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers.
- Undertaking periodic infection control training (minimum twice annually).
- Routine management of own team / team areas, and maintenance of work space standards
- Waste management including collection, handling, segregation, container management, storage and collection.
- Spillage control procedures, management and training.
- Decontamination control procedures, management and training, and equipment maintenance.
- Maintenance of sterile environments.
- Demonstrate due regard for safeguarding and promoting the welfare of children.
