Main duties of the job

Working in a community setting across the Cannock and/or Seisdon Geographic.

Delivering a high-quality clinical service to patients and their families.

Care coordination of an identified caseload with access to regular and robust supervision.

Delivering Physiotherapy assessment and interventions within a community setting to support service users to remain at home for longer.

Take responsibility for the management of a caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.

Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.

Assess patient's individual care needs, develop, implement and evaluate programmes of care and rehabilitation.

Deliver a range of non-pharmacological interventions following specific and thorough Physical assessments to enable understanding of any expression of an unmet need and to maximise an individual’s abilities.

An annual salary of up to £32,934 (37.5 hours per week) depending on experience

35 - 41 days annual leave – including bank holidays (depending on length of service)

Registered Physiotherapist

Experience of working with people who have difficulty in engaging with services and have complex health needs

Strong team-working skills

A high level of self-motivation and a flexible approach

A commitment to high-quality care and values and evidence-based practice

Excellent teamwork skills, and professionalism

You will be working primarily between Monday and Friday within the hours-of-service delivery 08:00-17:00.
