Job Summary:

The Clinical Pharmacist will be an integral member of the multidisciplinary healthcare team, working directly with patients and healthcare professionals to ensure the safe, effective, and economical use of medicines. Leveraging expert knowledge gained through training, the post-holder will primarily focus on medicines optimization and safety,chronic disease management, clinical medication reviews, and supporting the delivery of high-quality patient care in line with national and local guidelines.

Key Responsibilities:

1. Clinical Practice & Patient Care:

* Medicines Optimization: Conduct comprehensive structured medication reviews (SMRs) for patients with complex polypharmacy, multiple long-term conditions, and those in care homes, identifying and addressing medicine-related issues.

* Chronic Disease Management: Proactively manage patients with long-term conditions (e.g., diabetes, hypertension, asthma, COPD) in collaboration with GPs and practice nurses, including prescribing (if an independent prescriber) and monitoring.

* Acute Medicine Management: Respond to acute medicine requests, manage common ailments, and triage patient queries related to medication, signposting to other healthcare professionals as appropriate.

* Discharge and Transitions of Care: Facilitate medication changes and reconciliation for patients recently discharged from hospital, ensuring continuity of care and preventing medication errors.

* Medicines Information: Act as a primary point of contact for all medicine-related queries from patients and healthcare staff, providing evidence-based information and advice.

* Patient Education and Adherence: Provide tailored education and support to patients to improve understanding of their medicines, promote adherence, and encourage shared decision-making.

* Prescribing:Independently prescribe within scope of practice for various conditions, following relevant clinical guidelines and protocols.

* Clinical Skills: Utilise enhanced clinical skills (e.g., physical examination, consultation skills) to assess patients and inform medication management decisions.

2. Medicines Management & Safety:

* Repeat Prescribing: Oversee and manage the repeat prescription reauthorization process, ensuring timely and appropriate reviews, and promoting electronic repeat dispensing.

* Formulary Management: Contribute to the development and maintenance of practice and Primary Care Network (PCN) formularies to promote safe, high-quality, and cost-effective prescribing.

* Safety Alerts & Guidance: Implement changes to medicines in line with MHRA alerts, product withdrawals/shortages, and other local or national guidance.

Ensure monitoring of cqc safety searches on emis, and be familiar with online tools such as eclipse.

* Audit & Quality Improvement: Lead and participate in medicines-related audits, undertake quality improvement initiatives (e.g., PDSA cycles), and implement recommendations to enhance prescribing quality and safety.

* Prescribing Data Analysis: Interpret prescribing data to identify areas for improvement and support practices in achieving Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) targets.

3. Education, Training & Leadership:

* Training & Mentorship: Provide medicines education and training to GPs, nurses, and other healthcare professionals within the practice and PCN. May provide supervision or mentorship to junior pharmacists or pharmacy technicians.

* Collaboration: Foster strong working relationships with community pharmacy, hospital pharmacy, and other primary care professionals to facilitate integrated care.

* Policy Development: Contribute to the review and setting of policies related to medicines management and prescribing.

* Innovation & Research: Support clinical research and innovation within the practice/PCN related to medicines use.

4. Professional Development & Governance:

* Continuous Professional Development (CPD): Maintain own clinical and professional competence, actively engaging in CPD in line with GPhC requirements and CPPE recommendations.

* Clinical Supervision: Actively engage in clinical supervision with a designated GP or senior clinical pharmacist.

* Record Keeping: Maintain accurate, timely, and complete patient records using appropriate clinical templates and coding.

* Safeguarding: Adhere to all relevant safeguarding policies and procedures.

Person Specification:

Qualifications:

* Registered Pharmacist with the General Pharmaceutical Council (GPhC).

* Completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP) or equivalent primary care clinical pharmacist training.

* (Desirable) Independent Prescribing qualification (or working towards/willing to undertake).

Experience:

* Significant post-registration experience as a pharmacist, ideally within a patient-facing clinical role.

* Experience in conducting clinical medication reviews.

* Experience in managing patients with long-term conditions.

* Familiarity with primary care IT systems (e.g., EMIS Web, SystmOne).

Knowledge & Skills:

* Expert knowledge of medicines and therapeutics across a broad range of disease areas.

* Strong understanding of medicines optimization principles and strategies.

* Excellent communication, interpersonal, and consultation skills, with the ability to engage effectively with patients and healthcare professionals.

* Ability to work autonomously and as part of a multidisciplinary team.

* Strong analytical and problem-solving skills, with the ability to interpret complex clinical information.

* Ability to plan, manage, monitor, advise, and review general pharmaceutical care programmes.

* Knowledge of NHS structures, policies, and priorities, particularly within primary care.

* Proficient in using IT systems for patient records and data analysis.
