Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve, and we are looking for a Care Coordinator to join our team.

We are looking for a highly motivated Care Coordinator to lead the procurement of nursing home placements and/or domiciliary care for patients clinically assessed for Hospice Rapid Access Service.

The post will develop relationships both in-house and with external organisations, to ensure that the patient has access to the most appropriate care environment for their holistic needs. The candidate should manage timescales sensitively and have a good understanding of GDPR and confidentiality.

You will be a team player, motivated, forward thinking and a talented individual interested in delivering excellent specialist palliative care within the Hospice Rapid Access Service team.
