We're looking for a compassionate and motivated professional to support adults with a range of complex care and support needs to live independent, safe and fulfilling lives within their local communities. Working as part of a dedicated Adult Social Care team, you'll use a strengths-based approach to help people achieve their goals, maintain their independence and access the support they need to enhance their wellbeing.
In this varied and rewarding role, you'll manage a diverse caseload and work closely with individuals, families, carers and partner organisations to assess needs, develop personalised support plans and review outcomes. You'll undertake holistic assessments, consider risk and mental capacity, and ensure that support arrangements are tailored to each person's circumstances and aspirations.
No two days are the same. You may be responding to urgent situations where quick decision-making is needed to safeguard vulnerable adults, contributing to safeguarding enquiries, or coordinating services that enable people to remain safely within their communities. You'll also work with health colleagues and other agencies to ensure people receive the right support at the right time.
