Role Purpose
The SEEDS Project Worker will support the setup, implementation and roll out of the DEEDS social model of care in (area) to include the delivery of programmes and services for older adults living with dementia, support for their carers and work towards improving Dementia awareness across the wider community.
Working in partnership with the Healthy Living Centre Alliance SEEDS Project lead and the SEEDS Regional Manager the post-holder will provide frontline support, facilitate activities, and encourage engagement to help people living with Dementia and carers to improve wellbeing, maintain independence, reduce isolation, and remain connected within their communities.
The role requires a compassionate, person-centred approach alongside strong partnership working, community engagement, and organisational skills.
Key Responsibilities
Organisational Responsibilities
- Work as part of the Causeway Rural & Urban Network team to support the delivery of the organisation’s strategic priorities and objectives.

Focused Responsibilities
Programme Delivery and Facilitation
- Support the planning, coordination and delivery of DEEDS / SEEDS activities for people living with dementia and carers in community and group settings.
- Assist with the preparation, set-up and facilitation of sessions, workshops, events and activities.
- Deliver programmes that promote positive ageing, wellbeing, social inclusion, dementia awareness and community connection.
- Encourage active participation and engagement from programme participants.
Support for PWD and Carers
- Provide respectful, person-centred support to older PWD and carers participating in the programme.
- Build positive relationships with participants and provide encouragement to reduce social isolation and improve confidence and wellbeing.
- Provide practical, social support in line with programme guidance and organisational procedures.
- Support individuals to identify goals and contribute to the development of individual support plans where appropriate – suitability assessments, carers agreement.
- Assist with planned exit strategies and signpost participants and families to additional support services where required.
Participant Engagement and Signposting
- Support one-to-one engagement activities.
- Provide information on relevant community, voluntary and statutory services.
- Assist participants and carers to access additional support through agreed referral and signposting pathways.
- Promote awareness of DEEDS / SEEDS services within local communities.
Partnership and Network Working
- Develop and maintain positive working relationships with community, voluntary and statutory organisations.
- Support the coordination of local networks and partnerships contributing to DEEDS / SEEDS activities.
- Act as a point of contact for project partners, community groups and venues involved in programme delivery.
- Support outreach activity, awareness sessions and community events linked to the project – carers education
Volunteer Support
- Assist with the recruitment, induction, training and ongoing support of volunteers involved in programme delivery.
- Encourage positive volunteer engagement and participation.
Monitoring, Recording and Reporting
- Maintain accurate records of attendance, engagement and activities delivered.
- Input participant information onto agreed monitoring systems in line with organisational procedures.
- Support the collection of participant feedback, case studies and outcome information for monitoring and evaluation purposes.
- Contribute to reporting requirements for funders and organisational monitoring processes.
Safeguarding, Quality and Compliance
- Work in accordance with organisational safeguarding, confidentiality, health and safety, equality and data protection policies.
- Maintain professional boundaries and uphold high standards of conduct at all times.
- Report safeguarding concerns, risks or incidents promptly in line with organisational procedures.
- Ensure all activities are delivered in a safe, inclusive and respectful manner.
General Duties
- Promote equality, diversity, inclusion and dignity in all interactions.
- Participate in supervision, appraisal, training and team meetings.
- Undertake any other reasonable duties commensurate with the role.
Pay: From £26,590.00 per year
