We are recruiting an Assistant Practitioner to join our Planned therapy team within Integrated and Community Care Services at University Hospital Coventry and Warwickshire. Assistant Practitioners play a vital role in helping people live independent lives through assisting with therapy, holistic care and supporting connection within the persons community.
You will put people at the centre of every interaction; exploring what matters most to them through holistic assessments and provide therapeutic interventions. You will empower individuals to achieve their goals.

The role of therapy and therapy support staff is key to the successful delivery of this exciting and transformational service, aimed to improve the health and wellbeing of the Coventry population.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
To work as an Assistant Practitioner within the Planned Community Therapy Team as a member of a multidisciplinary team.
The post holder will provide rehabilitation to adults in Coventry with a wide range of conditions under the direction of a team of qualified therapists, in patients current residence, or other community settings
Assist and lead in supporting individualised treatment programmes assessing and reviewing outcome measures and supporting effective service development.
In addition to these responsibilities following training and completion of clinical competency documents the Assistant Practitioner will be able to deliver delegated therapy assessments and interventions with a level of autonomy equal to their qualification and competency.
As required, to work flexibly to ensure appropriate service provision over a 7 day working pattern.
To adhere to the lone worker policy.
There will be an on-going and daily requirement for the post-holder to travel to different locations within Coventry and Warwickshire
The post holder will be required to have a full UK driving licence & car and to be insured for business use (please note that an international driving license is not accepted).

To have knowledge and understanding of a wide range of conditions to be able to contribute to and undertake therapy assessment and treatment
To deliver delegated therapeutic assessment or treatment according to the patients’ goals and complexity, with co-workers or as a lone worker, according to established competency.
To regularly visit patients in their own homes and other community settings.
To assist in moving and handling and therapeutic positioning of patients to provide assessment and intervention. This may include assisting the patient getting up and down off the floor, managing stairs and working in confined spaces.
To progress an individual’s therapy programme autonomously, seeking professional guidance and supervision when required if scope of practice is exceeded.
Using equipment, prescribed for the individual within therapeutic tasks or sessions.
Ordering and assessing for basic therapy equipment with delegated patients following completion of assessed competency.
To demonstrate knowledge and skills in the use and application of specific equipment to enable the training of others (including patients, carers, relatives and staff) to a given competency.
To work collaboratively with Social Services and third sector providers to ensure delivery of effective safe care.
To complete holistic assessments of patient needs with a focus on enablement, home first and effective utilisation of resources
To work collaboratively with the team participating in evaluation and audits of the service and support in identifying opportunities for improvement and service development.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group. If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
