Full job description

We are seeking an Occupational Therapist to join our multidisciplinary Community Rehabilitation Team on a fixed-term basis. The post will suit an enthusiastic Occupational Therapist with an interest in developing skills across a wide range of therapeutic approaches.
Our Community Rehabilitation Team supports individuals with complex and enduring mental health needs to achieve recovery and independence within the community across the Pennine Care footprint. We are committed to delivering high-quality, evidence-based assessment and treatment, psychological care and fostering a culture of innovation, compassion, and collaboration.
This is an exciting opportunity to make a real difference in the lives of people with severe and enduring mental health difficulties.
The team is a trust wide service covering all five boroughs of Pennine Care and it is essential that you drive and have access to transport to travel across the whole footprint of the trust. We are currently based at Trust HQ in Ashton Under Lyne. Our core hours are Monday to Friday 9am-5pm.

To embed the Occupational Therapy assessment and intervention pathway across the Community Rehabilitation Team, within the framework of Model of Human Occupation in collaboration with the wider MDT.

To be responsible for a caseload of service users under the care of the Community Rehabilitation Team, supporting them in their recovery journey.

To undertake specialist OT assessments in all areas of self-care, productivity and leisure to support discharge planning and identify support needs.

To conduct OT assessments, developing outcome-based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence-based outcome measures.

To provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.

To develop treatment plans and intervention to be delivered by the team’s social inclusion workers.

To provide clinical supervision to the junior staff within the Community Rehabilitation Team.

To offer a range of OT assessments and interventions for service users within the Community Rehabilitation Team.
To be able to work across all functions within the and support patients to achieve their individually agreed goals.
To work as part of a multi-disciplinary team to, assess, plan and implement individualised programmes of care/interventions in accordance with CPA (where appropriate).
To lead on the development of group-based interventions to aid recovery and promote social inclusion.
To facilitate the development of peer support networks as part of the discharge pathway from the service.
To deliver a range of therapeutic interventions in line with professional requirements and in accordance with the job specification.
To write reports using MOHO outcome measures and to ensure up-to-date and accurate records are maintained regarding all patient contact.
To use a comprehensive range of verbal and non-verbal communication tools to communicate effectively with patients, their carers and families and colleagues. This may include people who may have difficulties with understanding or communication.
To be responsible for managing a caseload of clients as required.
To manage patient discharge in a safe and timely manner, ensuring liaison with relevant professionals, to ensure that family and carers are, where appropriate, advised of the patients discharge arrangements, with timely completion of documentation.
To develop partnership working with all relevant agencies/services/individuals to support optimal care for patients
To work in partnership, direct, support and supervise the team’s technical instructor.
