Hours: 30 hours per week

Working Days: Wednesday to Friday

Working Hours: Between 7:00am and 6:30pm to be agreed

We are seeking a motivated and organised Lead Care Navigator to join our team on a part-time basis. This is a key non-clinical role that will work alongside the current Lead Care Navigator to provide leadership and support to the Care Navigation Team, ensuring effective cover and service delivery across Monday to Friday.

The successful candidate will play a vital role in helping patients navigate the healthcare system by coordinating care, managing patient information, and ensuring individuals receive the appropriate support, resources, and guidance.

This is an excellent opportunity for an individual who is passionate about delivering high-quality patient support and contributing to the smooth running of healthcare service

Main duties of the job

Key Responsibilities:

Support and oversee the daily operation of the Care Navigation Team.
Provide guidance, leadership, and support to team members.
Coordinate patient care and signpost patients to appropriate services.
Maintain accurate and confidential patient records.
Communicate effectively with patients, healthcare professionals, and external agencies.
Ensure a high standard of patient service and administrative support.

About You:The ideal candidate will demonstrate:

Strong leadership, motivation and team-working skills.
Excellent organisational and time-management abilities.
A proactive and motivated approach to work.
Outstanding telephone and communication skills.
Previous administrative experience, ideally within a healthcare or customer service environment.
The ability to work effectively in a fast-paced setting while maintaining attention to detail.
