The post holder will work as part of a dynamic multi-agency integrated team assessing referrals regarding ICRAS Service requirements for patients across Liverpool and South Sefton.
The post holder will -
Ensure due regard to patient confidentiality the coordinator needs to interpret and share the
information that is necessary to promote ICRAS across organisations.
Be responsible for co-ordinating care delivered through the ICRAS model.
Support the delivery of ICRAS model and influence service development.
Have an ability to operate within complex health and social care system,
across Health and Social Care and third sector providers.
Provides leadership within ICRAS services.
Teach and present to groups of staff, changes in both ICRAS Service provision and direction and ensure change in policy is implemented across the whole system.
Coordinate interventions for patients, communication with GP practices, Secondary Care providers and collaboration with staff inside and outside the ICRAS model.
Take ownership on behalf of the ICRAS model for continuous quality improvement, innovation and integration.
Contribute to the development and take responsibility for the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
To ensure smooth running of respective ICRAS teams
To clinically review, coordinate and distribute referrals, directing staff accordingly.
To manage MDT meetings/SAFER huddles associated preparation and facilitation.
Coordination of Health Professionals outside of the core/ specialist services for input into patient
care or attendance of MDT.
Management of incidents and complaints.
To ensure the interface between the acute hospitals, primary care, community health services,
social care, housing, the independent sector, voluntary sector are robust at all times.
To maximise the capacity of ICRAS teams.
To ensure that ICRAS services are responsive to the needs of local population.
To engage with other health and social care systems around good practice for ICRAS and research
models of service.
Ensure ICRAS Care providers provide high quality care appropriate to client needs.
To ensure effective use of Integrated Care services across the Whole Systems
To support analysts in reporting of data, identifying information gaps and pressures within the whole ICRAS system. To ensure the standards of service are delivered in line with local and national guidelines across all professional groups.
Support line management and authorised signatory of identified accountable staff.
Support / training and development of ICRAS staff.

Management
The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders, from a wide range of services and disciplines.
The post holder will coordinate complex activities that span a range of disciplines and organisations both NHS and non –NHS.
Record, interpret and present health information and issues that can impact on the risk or needs assessment of patients from a range of health perspectives. This may involve disclosing relevant
information on the patient or carers in ensuring the information is processed fairly and proportionally.
Develop and implement data collection systems and ICRAS protocols that will provide accurate and timely data safely and effectively and within the bounds of confidentiality and the data protection
act.
Disseminate and communicate decisions and appropriate health information to relevant health professionals including primary and secondary care for sharing best practice.
Identify service gaps and issues relating to effective clinical pathways and /or cross boundary arrangements.
Escalate evidence of ineffective arrangements within or outside of the ICRAS.
Records appropriate information about health outcomes on the appropriate recording system on a case-by-case basis.
Participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice and expertise.
The post holder will carry out any other duties as may reasonably be required by their line manager.
Leadership
Contribute to performance improvement of the ICRAS taking the lead for identified areas where agreed by the team
Provide coordination of and participate in relevant internal and external working groups and provide advice, expertise and support where requested.
Provide relevant and timely advice and reports on the function of the ICRAS to Urgent and Intermediate Care service line within Community Division
Work with members of the ICRAS team to investigate the causes of any variance in ICRAS activity and proactively contribute to the implementation of solutions.
Participate and lead specific working groups in order to achieve targets and objectives in relation to ICRAS
Proactively participate to identify risks and issues with regards to the functions of ICRAS teams to facilitate resolution and /or escalation processes.
Clinical
Act as a resource on matters of ICRAS care for other disciplines, agencies and the general public.
Prioritise and manage time and workload effectively, informing line manager about workload constraints affecting services.
Be responsible for identified team members in their role, responsibility and professional development.
Provide within the team, support and advice in areas of professional expertise.
Support staff in the team in developing skills within the Scope of Professional Practice.
The post holder shall as necessary provide cover for and undertake duties of absent care coordinator
colleagues across Liverpool and South Sefton ICRAS teams.
The post holder shall follow all the policies and procedures for the organization.
Provide clinical leadership as appropriate.
Non Clinical
Provide regular reporting and monitoring from a quality and safety perspective.
Contribute to the strategic planning of ICRAS team projects, identifying interdependencies across
projects/functions and services, potential impacts of integrated care activity on wider organisation,
resource requirements and building in contingency and adjustments as necessary.
Contribute to the development of performance and governance strategies and the development and implementation of integrated care improvement programmes in accordance with Trust
priorities.
Contribute to short, medium and long term service developments, for ICRAS to develop and achieve quality outcomes.
Manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales.
Use computer software to create reports for other professionals and meetings as required. Develop policies and procedures in own work function with an impact on the wider organisation, as required.
Contribute to the drawing up of the ICRAS team budget.
The post holder will hold a designated budget and be responsible for the purchase of physical assets and supplies.
Training & Education
Participate in an individual performance review on an annual basis and be proactive in Continuous
Professional Development.
Utilise informal and formal learning opportunities both independently and with others.
Take a lead in the coordination and provision of training and development, to ensure a wide understanding of the ICRAS.
Provide internal and external training development of the ICRAS team.
Practice in accordance with the professional, ethical and legal codes of the Nursing and
Midwifery Council and Mersey Care NHS Foundation Trust protocols and guidelines.
