Full job description

EXPERIENCE ESSENTIAL

Part-time, SystmOne trained Care Coordinator for 20 hours per week at Watton Place Clinic - a friendly village surgery in Watton at Stone with approximately 5,000 patients.

Please do not apply if you are living in London as it is too far to travel.

Job Description:

· Target driven

· Use Ardens Manager to Identify patients on QOF/ECF/NCDES

· Ability to work to achieve targets on QOF/ECF/NCDES via Ardens Manager

· Identify and work with patients suffering from asthma, depression and LTC’s to support their personalised care requirements using available decision support aids. This includes using scoring systems and templates in Ardens and S1

· Knowledge of Accurx to send invites and links to patients

· Able to find and run reports in S1

· Understand the importance of read coding in S1

· Identify patients eligible for NHS cancer screening programmes – cervical, bowel and breast and create and maintain a robust system to chase up non attenders

· Offer face to face appointments and complete appropriate elements of the QOF and Enhanced Funding Framework (ECF).

· Ensure patients have good quality information to assist them in making choices about their care

· Active involvement in development and delivery of flu, Covid, RSV and other vaccine campaigns

· Participate in Teamnet, local education meetings, QOF, ECF & Ardens training

Who you are:

Target driven

Be confident in your use of SystmOne, creating searches and completing Ardens templates

Excellent organisation skills

Be an empathetic communicator with good listening skills and be able to explain and simplify concepts to patients

Have experience of working in a team

Be enthusiastic about health promotion and prevention programmes

Watton Place Clinic is a rural dispensing surgery in Hertford & Rurals PCN primarily serving the villages around Watton at Stone. Serving 5,000 patients, we have three doctors, two nurses, and a small admin team.

Salary to be negotiated, dependent upon experience
