Full job description

This post is a Fixed Term/ Secondment until 31.03.2028. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

Are you an enthusiastic and motivated dietitian with expert skills in Diabetes Dietetics? Are you passionate about helping individuals with Type 2 Diabetes achieve remission via evidenced based dietary and lifestyle approaches?

An exciting opportunity has arisen to join the well-established dynamic diabetes dietetic team at Hywel Dda which holds a high profile both within the wider therapies team and across the Health Board. We fully support continuing professional development, appraisal and supervision and actively enable this with protected sessions in individualised job plans.

This is a key role for a lead in Type 2 Remission: Di abetes Dietetics, involving both clinical and leadership responsibilities. This role offers a unique opportunity for someone with a passion to improve health outcomes across Hywel Dda and a commitment to develop and learn. Y ou will have the opportunity to be a valuable part of the friendly, supportive diabetes service health board wide.

You will:
deliver expert level clinical Diabetes Dietetics, managing a complex caseload of patients with Diabetes focusing on those individuals looking to achieve Type 2 diabetes remission.

Manage a diabetes caseload, informing multi-disciplinary care (inc. Primary care and stakeholders) and provide expert support and clinical supervision for dietitians / support workers / practice based learners/ administrators Health Board wide.

Clinically lead the development, implementation, delivery and evaluation of best practice in relation to adult Type 2 Diabetes remission dietetics, Health Board wide.

Work as an integral member of the adult diabetes multi-professional leadership team, driving excellent nutritional care through evidence based practice.

Ensure seamless service planning and delivery from promotion of service, recruitment of service users and evaluation of intervention.

Clinically lead the development and delivery of education and training in relation to diabetes working across Multi profession platforms and multi-disciplinary teams (MDT) to support optimal effective diabetes care.

Compassionately and effectively line manage members of the diabetes dietetic team.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
