Oxford Health NHS Foundation Trust is looking for compassionate, motivated and resilient Senior Support Workers to join our newly established Intensive Case Management and Rehab Pathway .
This innovative service has been created to support people living with severe and enduring mental health conditions who often find it difficult to engage with traditional community services. Working as part of a multidisciplinary team, you'll help people rebuild their lives by providing flexible, assertive and recovery-focused support where it's needed most.
Unlike traditional office-based services, this role is centred on outreach. You'll meet people in their homes, communities and other settings, building trusting therapeutic relationships and delivering practical interventions that make a genuine difference. The service takes a proactive approach, responding quickly to changing needs and supporting people to remain well, maintain their independence and avoid unnecessary hospital admissions.

As a Senior Support Worker, you will work under the supervision of registered clinicians to support people with complex mental health needs, including those who may also experience homelessness, substance misuse, social exclusion or difficulty engaging with services. You'll work alongside nurses, occupational therapists, psychologists, psychiatrists and social care colleagues to deliver person-centred care that promotes recovery and independence.
Key responsibilities include:
Building therapeutic relationships with service users and carers.
Supporting people to engage with care plans and community services.
Providing practical support to attend appointments, access housing, benefits and community resources.
Identifying early signs of relapse and escalating concerns appropriately.
Supporting patients during periods of crisis alongside registered clinicians.
Delivering simple psychological interventions under supervision.
Maintaining accurate clinical records and contributing to MDT meetings.
Working flexibly across community settings throughout Buckinghamshire.

You'll be joining a newly established, fully integrated multidisciplinary team that's committed to delivering innovative, person-centred mental health care across Buckinghamshire.
