Job summary:
We are looking for an experienced healthcare practitioner to join our thiving and talented team for Saturday and/or Sundays. As the successful candidate, you will have a background in acute and urgent care, ideally have completed an MSc in Advanced Practice, and must be independent prescribing. You will also be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced clinical practitioners will work across the four pillars of advanced practice; clinical, leadership, research and education.
As the post-holder, you will practice as an Advanced Nurse Practitioner across various services, including but not limited to ABC Remote Services (uCATS, Enhanced Access, Additional Primary Care, Smoking Cessation and ADHD Primary Care Connect). As such, you will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.
The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Training will be provided to equip the post holder to manage the prescribing of smoking cessation medication as part of a collaborative service with West Sussex County Council.
Whilst the role is fully remote, you would be required to attend our Horley office to collect IT equipment and attend meetings on a quarterly basis. We have 12-hour shifts available on Saturdays and Sundays.
Main duties of the job:
- To independently undertake specialised clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
- To ensure patients are fully involved in the design and implementation of their treatment plans.
- To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.
- To undertake assessments as part of the ADHD Primary Care Connect service, diagnosing and prescribing medication as appropriate and with the support of the Consultant Psychiatrist
- To prescribe smoking cessation medication in line with service protocols
- To refer appropriately to clinical leads/supervising GP/Consultant Psychiatrist where a second opinion, or supervision, is required.
- To adapt styles of communication appropriately to promote empowerment, self-management, and health education for patients.
- To attend clinical case conferences, MDTs, review and clinical meetings as required.
- To be a source of clinical expertise to less experienced staff within the team.
- To delegate duties/tasks to other members of staff where appropriate.
- Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relative, and carers.

Leadership Service Delivery Responsibilities
- To ensure that patient records and documentation are maintained in line with recognised standards and ABC policies and use of the electronic patient records system. This includes
- Documenting full and accurate record of assessments, investigation, procedures, treatment, and aftercare.
- Supporting colleagues to maintain good and complete records.
- Working closely with the administration and/or operations team, ensuring all records pertinent to patients are filed as appropriate.
- Contributing to the read-coding of patient data.
- Work within local and national guidelines and protocols
- To participate in audits and use the results, current research and government guidelines, to make recommendations for changes to service delivery and clinical practice, and to share knowledge with other healthcare professionals.
- To contribute to the maintenance and improvement of communication networks with all disciplines to ensure continuity of care.
- To collect data and provide service information to the Service Manager as required.
- To contribute to the development and establishment of systems to gather feedback from patients.
