Key Worker / Support Worker – 18+ Supported Accommodation

OTS Services Limited is seeking a dedicated, compassionate and motivated Key Worker / Support Worker to join our team supporting adults aged 18 and over within our supported accommodation services.

The successful candidate will play an important role in supporting individuals to develop independence, maintain their wellbeing and achieve their personal goals. You will work closely with allocated individuals as their Key Worker, ensuring that their support is person-centred, outcome-focused and appropriately documented.

The Role

As a Key Worker, you will provide practical, emotional and structured support to adults living in supported accommodation. You will be responsible for monitoring the progress of allocated individuals, maintaining accurate records and contributing to the ongoing review of their support needs.

You will work closely with colleagues, management and relevant professionals to ensure that individuals receive consistent and appropriate support.

Key Responsibilities

Provide person-centred support to adults aged 18+ within supported accommodation.
Act as a designated Key Worker for allocated individuals and develop positive, professional working relationships.
Support individuals to develop and maintain independent living skills, including budgeting, cooking, shopping, cleaning, managing appointments and maintaining their accommodation.
Support individuals to identify personal goals and work towards agreed outcomes.
Monitor and record progress against individual support plans and agreed outcomes.
Complete accurate daily records, case notes, handover records and incident reports.
Prepare and contribute to weekly Key Worker reports and monthly progress reports, ensuring that these accurately reflect the individual's progress, engagement, achievements, concerns and ongoing support needs.
Contribute to regular support plan and risk assessment reviews, ensuring that information remains current and reflects the individual's circumstances.
Identify and report any changes in an individual's presentation, behaviour, wellbeing or support needs to the appropriate manager.
Record and report safeguarding concerns, incidents, accidents and other significant events in accordance with company procedures.
Support individuals to attend appointments and access education, employment, training, healthcare and community services.
Encourage participation in meaningful activities that promote independence, confidence, social inclusion and wellbeing.
Maintain professional boundaries and promote dignity, choice, privacy and respect at all times.
Liaise appropriately with management, social workers, healthcare professionals and other relevant agencies.
Attend and contribute to team meetings, supervisions, reviews and other meetings as required.
Ensure all documentation is completed accurately, professionally and within the required timescales.
Follow company policies and procedures, including safeguarding, health and safety, confidentiality, data protection and incident reporting.
Maintain a safe, clean and supportive environment for the individuals we support.
Undertake other reasonable duties associated with the role as required by management.

About You

We are looking for someone who is caring, reliable, professional and committed to supporting adults to achieve greater independence. You must be able to communicate effectively, maintain professional boundaries and demonstrate good judgement when responding to concerns or changes in an individual's circumstances.

You should also be confident in writing clear, factual and professional reports.

Essential Skills and Requirements

Experience in support work, supported accommodation or a similar environment is desirable.
Experience of working with adults aged 18+ is desirable.
Good understanding of person-centred support and promoting independence.
Good written and verbal communication skills.
Ability to produce clear and accurate reports and maintain professional records.
Good organisational and time-management skills.
Ability to work independently and as part of a team.
Ability to maintain professional boundaries.
Commitment to safeguarding, equality, diversity and inclusion.
Flexibility to work in accordance with the needs of the service.
Applicants must have the right to work in the UK.
Satisfactory references and relevant pre-employment checks will be required.
An Enhanced DBS check is essential for this role, and applicants should have a valid Enhanced DBS registered on the DBS Update Service for working with adults, or be willing to undergo the required check as part of the onboarding process.
