Full job description

An opportunity has arisen to join a highly regarded and established team based at Hinchingbrooke Hospital, at a time of growth and change.

We will recruit an enthusiastic and motivated Clinical N urse S pecialist or Social Worker to join our Psychiatric Liaison T eam .

We are particularly keen to hear from applicants with experience of working across age ranges .

To assist in the delivery of a high-quality Liaison Psychiatry Service within Hinchingbrooke Hospital in keeping with the visions and values of the Cambridgeshire and Peterborough NHS Foundation Trust.

Providing and undertaking timely psychosocial assessments and risk management plans in accordance with CPFT’s Clinical Risk Assessment and Management policy for adults presenting to the emergency department with mental health problems.

Promoting a safe and effective service throughout ED; this will be through training and giving expert advice and support to nursing and medical teams in ED.

Maintaining a visible, professional presence where patients, carers and staff can turn for assistance, advice and support.

As part of the Liaison Psychiatry Service, the post-holder will contribute to the multidisciplinary clinical assessment and management of patients referred to the service in collaboration with other team members.

To participate in and promote close links with community mental health teams and other agencies to develop effective care plans.

1. To conduct Biopsychosocial and risk assessments for individuals presenting in the Emergency Department with mental health problems and admitted to inpatient wards at Hinchingbrooke Hospital.

2. Provide a high level of clinical expertise in the assertive management and assessment and care of patients presenting with mental health problems.

3. Ensure that care is based on the assessment of care needs, and includes a thorough risk assessment and risk management plan.

4. Ensure good communication with other members of the Liaison Psychiatry Service as well as general hospital staff about changes in care plans.

5. To liaise with other agencies caring for patients, including community mental health teams and GPs to provide clear plans for all patients, particularly frequent attending or high risk patients.

6. Utilise recent research findings in the advancement of best practice and in the delivery of an evidenced based service.

7. Engage in Liaison Psychiatry Service educational and training events to fulfil requirements of clinical governance.

8. To organise and provide training in psychiatry, including mental health awareness.

9. To provide support and expert advice to colleagues involved in the care of patients with mental health problems and to develop systems to ensure that this support is available in a range of ways.

10. Ensure that the requirements of the Mental Health Act 1983 and other statutory regulations are observed and implemented within a medical setting.
