Rota Scheduler / Care Coordinator (Domiciliary Care) – Office Based (Bexhill-on-Sea)

Care To You Healthcare supports adults to live safely and independently in their own homes across East Sussex and West Sussex. We are looking for an experienced, organised Care Co-Ordinator to join our team at our Bexhill-on-Sea office and take day-to-day responsibility for coordinating care rotas using our electronic care system, including Nourish.
The role
As our Care Co-Ordinator, you will plan and maintain daily and weekly care rotas to ensure every visit is covered safely, carers are matched appropriately (skills, location, continuity and client preferences), and changes are communicated clearly and promptly. You will work closely with the Care Manager and wider office team to support continuity of care, rota stability and a reliable service for clients and families.
This is a fast-paced role that suits someone who can work independently, stay calm under pressure, and make sound decisions while balancing staff availability, travel time and client needs. You may also be asked to support the management team with related coordination and administrative tasks to help the service run smoothly.
Key responsibilities
- Create, maintain and optimise daily and weekly rotas for clients across East Sussex and West Sussex
- Ensure all client visits are covered, identifying gaps early and resolving them promptly
- Match carers to clients based on skills, training, location, continuity and preferences
- Respond to short-notice changes (sickness, emergencies, annual leave) and reassign calls as required
- Update schedules and visit records accurately on Nourish (and any associated systems), ensuring records are complete and up to date
- Liaise with carers regarding rota changes, additional calls and emergency cover
- Communicate clearly and professionally with carers, clients, families and the office team
- Communicate and coordinate with members of the wider health and social care community, including health professionals and Adult Social Services, as required to support safe and effective service delivery
- Monitor and escalate concerns about missed visits, late calls or patterns of risk in line with company procedures
- Support safe staffing levels and effective use of resources (including travel time and realistic workloads)
- Maintain confidentiality and handle information securely in line with company policies
- Provide general coordination support to the domiciliary care office team as required, in line with the needs of the service
On-call requirement
You will participate in our on-call rota to support service continuity outside standard office hours, in line with our on-call arrangements and procedures.
About you
We are looking for someone who:
- Has experience in care coordination, rota scheduling or workforce planning (care sector experience is highly desirable)
- Is confident using electronic systems and comfortable learning new software (Nourish experience is an advantage)
- Can work independently, use initiative and make decisions within agreed guidelines
- Has excellent organisation, time management and attention to detail
- Remains calm, professional and solution-focused under pressure
- Communicates clearly and respectfully with colleagues, carers, clients and families
- Understands the importance of reliability and continuity in domiciliary care
